研究目的：

探讨五音疗法联合渐进式肌肉放松训练对乳腺癌术后患者疼痛综合征发生率、疼痛程度、上肢功能和生活质量的影响。  

Objectives of Study：

To explore the effects of five-tone therapy combined with relaxation training on the incidence of post-mastectomy pain syndrome, pain degree, upper limb function and quality of life in breast cancer patients after surgery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①组织病理学确诊为单侧原发性乳腺癌； ②年龄≥18岁； ③已行乳腺癌手术的患者，且已存在中重度疼痛，数字评定量表评分＞3分； ④无视听、认知障碍，且无从事音乐相关行业工作经历者； ⑤具有正常语言表达能力，能准确描述疼痛程度； ⑥自愿参加研究且书面签署知情同意书。

Inclusion criteria

① Unilateral primary breast cancer was confirmed by histopathology. ② Age ≥18 years old; ③ Patients who had undergone breast cancer surgery and had moderate to severe pain, numerical rating scale score > 3; ④ Ignoring listening and cognitive impairment, and having no working experience in music related industry; ⑤ With normal language expression ability, can accurately describe the degree of pain; ⑥ Voluntarily participate in the study and sign the informed consent form in person.

排除标准：

①术前存在乳房、肩部、胸壁、上臂等区域的慢性疼痛病史； ②有严重心、肝、肾功能障碍或其他恶性肿瘤病史； ③乳腺癌复发、转移； ④术后使用镇痛泵或镇痛药； ⑤术后发生并发症，如皮下积液或感染者； ⑥行乳房重建术者。

Exclusion criteria：

① History of chronic pain in breast, shoulder, chest wall, upper arm, etc.; ② History of severe heart, liver, kidney dysfunction or other malignant tumors; ③ Breast cancer recurrence and metastasis; ④ postoperative use of analgesic pump or analgesics; ⑤ Postoperative complications, such as subcutaneous effusion or infection; ⑥ Patients undergoing breast reconstruction.

引流管护理：包括引流管的妥善固定及液体的色、质、量的观察，伤口处敷料的护理等； 生活护理：对患者进行饮食指导、患肢功能锻炼及心理指导等; 疼痛护理：进行疼痛知识宣教，指导患者对疼痛的部位、程度以及持续时间进行评估，指导患者转移注意力以缓解疼痛，耐心倾听患者对疼痛管理的疑问，给予详细解答；上肢功能锻炼：指导患者术后进行患侧上肢功能锻炼。

研究者于术后第3日在病房中对患者进行五音疗法联合放松训练，每天2次，每次20min，连续4周。第一次和第二次训练由研究者在旁指导，同时研究者需随时观察患者生命体征变化。患者一旦出现不适，立即嘱其停止训练，症状严重者立即通知医师并协助处理。指导结束后让患者自行放松训练一次，确保患者已成功掌握放松训练的方法。第三次训练开始由患者自行训练，研究者在旁监督。患者居家期间每天跟随指导语进行训练，并且在训练后填写自行训练记录表。研究者每周对居家患者进行2次电话随访。在术后4周研究结束时，研究者统一回收患者的记录表。

Intervention：

Five-tone therapy combined with relaxation training was performed in the ward on the third day after surgery, 20 minutes each time, twice a day for 4 consecutive weeks. The first and second training sessions were supervised by the investigators, who monitored the vital signs of the patients at any time. If the patients felt uncomfortable, they were instructed to stop the training immediately, and if the symptoms were serious, the doctor was immediately notified and assisted. After the instruction, the patients were allowed to relax themselves once to ensure that the patients had successfully mastered the relaxation training method. The third training session began on the patient's own, with investigator supervision. The patients trained with the instructions every day at home, and filled in the self-training record form after the training. Home-based patients were contacted by telephone twice weekly. At the end of the study, 4 weeks after surgery, the patients' record forms were retrieved.
