研究目的：

本课题将观察推拿治疗腰椎间盘突出症（LDH）的临床疗效，探讨推拿治疗LDH的疗效与安全性。并借助静息态fMRI技术，从影像学角度探讨论LDH的中枢机制，推拿治疗LDH的脑功能机制，为推拿治疗LDH提供依据。  

Objectives of Study：

This topic will observe the clinical curative effect of massage on lumbar intervertebral disc protrusion (LDH) , and explore the curative effect and safety of massage on LDH. The central mechanism of LDH and the brain function mechanism of massage in the treatment of LDH were discussed with the help of resting fMRI.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

健康人：①右利手；②年龄18-60周岁；③无腰椎间盘突出症病史；④入选前至少1个月无任何疾病引起的疼痛病史且未接受任何与疼痛有关的治疗。⑤贝克抑郁量表第 2 版（BDI-Ⅱ）评分≤13 分；⑥贝克焦虑量表（BAI）评分＜45 分； ⑦受试者本人同意参与本研究，签署知情同意书； 腰椎间盘突出症患者：①右利手；②年龄18-60周岁；③既往CT或MRI检查显示L4- L5和/或L5-S1椎间盘突出④以神经根性疼痛为主要表现，且影像学与神经定位相符；⑤神经病理性疼痛评估量表（DN4）评分≧4分；⑥近一个月未服用止痛药以及营养神经药，未进行系统治疗者；⑦受试者自愿参加临床试验，并签署知情同意书。

Inclusion criteria

Healthy People:① right-handed; ②aged 18-60 years; ③no history of lumbar disc herniation; ④ no history of pain caused by any disease and no treatment related to pain for at least 1 month before enrollment; ⑤ Beck Depression Inventory 2nd Edition (BDI-II) score ≤ 13 points; ⑥ Beck Anxiety Inventory (BAI) score<45 points; ⑦ The subject agrees to participate in this study and signs an informed consent form; Patients with lumbar disc herniation: ① right-handed; ②aged 18-60 years; ③previous CT or MRI examinations revealed herniated discs at L4-l5 and/or L5-S1; ④presented mainly as nerve root pain, and the imaging is consistent with nerve localization; ⑤ the neuropathic pain assessment scale (DN4) ≥ 4score ; ⑥the patients who had not taken analgesics and neurotrophic drugs in the past one month and had not received systematic treatment; ⑦ the subjects volunteered to participate in the clinical trial and signed the informed consent.

排除标准：

①髓核突出明显压迫脊髓或马尾神经，进行性神经功能缺损； ②合并腰椎峡部裂或伴有中重滑脱者； ③合并其它腰椎重大疾病，如肿瘤、结核、严重骨质疏松等； ④外伤、手术导致的腰椎急性损伤疼痛、下肢神经损伤； ⑤有引起下肢神经病变的任何其他情况（糖尿病、神经系统疾病）； ⑥伴有心血管、肺、肾、造血系统等重大疾病者； ⑦精神病患者； ⑧孕妇或哺乳期妇女； ⑨存在功能磁共振检查禁忌症。

Exclusion criteria：

①the nucleus pulposus protruding obviously oppressed the spinal cord or cauda equina, resulting in progressive nerve functional defect, ②the patients with spondylolysis or moderate or severe spondylolisthesis, ③complicated with other major diseases of lumbar spine, such as tumor, tuberculosis, severe osteoporosis, etc. ④Acute injury and pain of lumbar vertebrae and nerve injury of lower limbs caused by trauma and operation; ⑤Any other condition (diabetes, neurological disorder) that causes neuropathy in the lower extremities; ⑥Patients with cardiovascular, lung, kidney, hematopoietic system and other major diseases; ⑦patients with psychosis; ⑧pregnant or lactating women; ⑨patients with contraindication of fMRI.
