研究目的:

完善的术前规划和实时术中指导是实现精准肝切除术的必要前提。为了实现这一目标,研究者研发了新型的3D打印肝脏模型,该模型利用4-丙烯酰啉(ACMO)和甲氧基聚(乙二醇)丙烯酸酯(mPEGA)共聚而成的物理交联自修复高分子材料打印形成。模型拥有出色的自修复能力,进行切割后,在室温下几分钟内有效地恢复原有结构。 本研究中,研究者拟评估通过反复试错的方法使用3D打印肝脏模型进行术前规划的可行性。同时,研究者拟进一步明确将3D打印模型整合到传统方法(增强CT或MRI)中是否可以提高肝切除手术的安全性和有效性。  

Objectives of Study:

Comprehensive preoperative planning and real-time intraoperative guidance are essential prerequisites for achieving precise liver resection. In pursuit of this goal, the investigators have developed innovative 3D printed liver models utilizing a physically crosslinked self-healing elastomer created through the copolymerization of 4-acryloylmorpholine (ACMO) and methoxy poly (ethylene glycol) acrylate (mPEGA). These printed models exhibit exceptional healing capabilities, efficiently restoring their structure within minutes at room temperature, and rapidly recovering within moments after being incised. Herein, the investigators aim to assess the viability of employing these 3D printed liver models as instrumental tools in designing the optimal surgical approach through an iterative trial-and-error methodology. Concurrently, the investigators aim to determine whether the integration of these 3D printed models into conventional methods (contrast-enhanced CT or MRI) can enhance the safety, ease, and efficiency of hepatic resection procedures.

纳入标准:

1.18-80岁; 2.可切除肝癌; 3.在本中心进行增强CT/MRI检查; 4.美国东部肿瘤协作组体力评分:0分; 5.Child-Pugh肝功能分级:A级; 6.实验室检查指标达到以下标准,病人能够耐受手术:血红蛋白≥90g/L, 中性粒细胞计数≥1.5×10?/L, 血小板计数≥100×10?/L, 谷草/谷丙转氨酶≤5正常值上限(ULN), 碱性磷酸酶≤2.5 ULN, 血清白蛋白≥30g/L, 血清肌酐<1.5 ULN, 国际标准化比值≤2 或凝血酶原时间延长≤6s, 肌酐清除率≥60 mL/min。

Inclusion criteria

1.18-80 years old; 2.Patients with a resectable tumor in the liver; 3.Patients with contrast-enhanced CT/MRI in our center; 4.Eastern Cooperative Oncology Group Performance status score: 0; 5.Child-Pugh classification: A; 6.The Laboratory test results meet the following criteria and patients can tolerate surgery: Haemoglobin≥90g/L, Neutrophil count≥1.5×10?/L, Platelet count≥100×10?/L, Aspartate or alanine aminotransferase≤5 upper limits of normal(ULN), alkaline phosphatase≤2.5 ULN, Serum albumin≥30g/L, serum creatinine<1.5 ULN, International normalized ratios(INR)≤2 or Prothrombin time(PT)exceed ULN≤6s, Creatinine clearance≥60 mL/min

排除标准:

1.伴有肝外转移; 2.术前28天接受过抗肿瘤治疗,如放疗,射频消融; 3.招募前3个月内出现临床显著出血或者出血倾向,或正在使用溶栓或抗凝药物; 4.严重肺部疾病(如肺纤维化,间质性肺炎),难以控制的糖尿病(空腹血糖≥10mmol/L); 5.其它不适合参加临床试验的疾病,如患有精神类疾病或者酒精依赖。

Exclusion criteria:

1.Patients with extra-hepatic metastasis; 2.Anti-cancer therapy or surgery such as radiotherapy, radiofrequency ablation in 28 days prior to the surgery; 3.Clinically significant bleeding or bleeding tendencies within 3 months prior to enrollment or on thrombolytic or anticoagulant therapy; 4.Severe lung disease (eg, pulmonary fibrosis that affects lung function, interstitial lung disease), uncontrolled diabetes mellitus (fasting blood glucose ≥10 mmol/L); 5.There are other unsuitable candidates for clinical trials, such as mental illness or alcohol dependence.

采用随机数字法进行 1:1 随机分组,使用 SPSS 进行,研究对象按照入组顺序编号,每个研究对象产生一个随机数字,随机数字按照从小到大排序,随机数字前 10 例分配至术前规划组,随机数字后 10 例分配至常规手术组。

Using the random number method for 1:1 random grouping, SPSS was used. The study subjects were numbered in the order of enrollment, and each study subject generated a random number. The random numbers were sorted from small to large, with the top 10 randomly assigned to the preoperative planning group and the bottom 10 randomly assigned to the conventional surgery group.
