研究目的：

⑴构建针对老年肝硬化患者的基于达标理论的阶梯式护理干预方案。 ⑵探讨基于达标理论的阶梯式护理干预方案在老年肝硬化患者中的可行性。 ⑶评价基于达标理论的阶梯式护理干预对老年肝硬化患者自我管理能力、焦虑抑郁水平以及并发生发生率的影响。  

Objectives of Study：

(1)To construct a step nursing intervention scheme for elderly patients with cirrhosis based on the criterion theory. (2)To explore the feasibility of step nursing intervention scheme based on standard theory in elderly patients with cirrhosis. (3)To evaluate the effects of stepwise nursing intervention based on the criterion theory on self-management ability, anxiety and depression level and co-occurrence rate of elderly patients with cirrhosis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）年龄≥60岁； 2）根据2019年修订的肝硬化诊治指南，确诊为肝硬化的患者； 3）无精神疾病及认知障碍患者； 4）自愿参加本研究并签署知情同意书； 5）有一定的认知读写能力，能够理解或经研究者解释后理解问卷含义。

Inclusion criteria

1) Age ≥60 years old; 2) Patients diagnosed with cirrhosis according to the 2019 revised Guidelines for the Diagnosis and Treatment of cirrhosis; 3) No mental illness or cognitive impairment; 4) Voluntarily participate in the study and sign the informed consent; 5) Have certain cognitive reading and writing ability, and can understand the meaning of the questionnaire after interpretation by the researcher.

排除标准：

1）意识不清，或有沟通障碍的患者； 2）合并有心、肺、肾等严重原发性疾病； 3）同时参与其他研究者； 4）合并有肝性脑病、急性上消化道出血、肝癌。

Exclusion criteria：

1) Patients with unclear consciousness or communication difficulties; 2) Complicated with serious primary diseases such as heart, lung and kidney; 3) Participate in other researchers; 4) Complicated with hepatic encephalopathy, acute upper gastrointestinal hemorrhage, liver cancer.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

患者住院期间的资料，由患者填写完毕后，资料收集员现场回收问卷并进行数据分析。患者无法填写时，可由家属指导填写。住院期间，由课题组成员责任护士指导填写各调查问卷。出院后，通过填写“问卷星”或者门诊随访的方式收集量表信息及并发症指标数据。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The data of the patients during hospitalization were completed by the patients, and the data collector collected the questionnaires on site and analyzed the data. If the patient is unable to fill in, the family member can guide the filling in. During hospitalization, the responsible nurses of the research group guided to fill in the questionnaires. After discharge, scale information and complication index data were collected by filling in "questionnaire star" or outpatient follow-up.
