研究目的：

建立糖尿病合并慢性肾脏病患者队列及生物样本库，利用多组学方法获得糖尿病肾病组学图谱，寻找新型生物标志物，对糖尿病肾病进行早期诊断、早期预后评估，指导精准分型治疗。  

Objectives of Study：

Establishment of a cohort and biobank for patients with diabetes complicated by chronic kidney disease, utilizing multi-omics approaches to obtain a comprehensive understanding of the diabetic kidney disease (DKD) omics landscape. The aim is to identify novel biomarkers for early diagnosis and prognosis assessment of DKD, guiding precision typing and treatment strategies.

纳入标准：

1.糖尿病合并慢性肾脏病的患者；2.获得患者知情同意，愿意参加此研究。

Inclusion criteria

1.Patients diagnosed with diabetes complicated by chronic kidney disease; 2.Obtained informed consent from patients who are willing to participate in this study.

排除标准：

1.合并其他类型肾病；2.预计生存期＜5年；3.合并恶性肿瘤。

Exclusion criteria：

1.Concomitant with other types of kidney disease; 2.Estimated survival period less than 5 years; 3.Patients with concomitant malignancy.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

基于华西医院大数据平台，建立肾脏内科糖尿病肾病专病数据库，临床实验室检查数据、患者基本信息均可通过医院大数据平台同步。通过大数据平台建立相应的病例记录表（CRF）及随访时间节点开展病人的随访管理。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Based on the big data platform of West China Hospital, the database of diabetes of kidney diaease in the department of nephrology was established, and clinical laboratory examination data and basic patient information could be synchronized through the big data platform of the hospital. The corresponding case record table (CRF) and follow-up time node were established through the big data platform to carry out follow-up management of patients.
