研究目的：

探索不同类型奖赏性康复训练对脑卒中患者的康复动机的影响。建立可行性更高、效率更高的的奖赏性康复训练及神经干预技术。 探索不同类型奖赏性康复训练对脑卒中患者的运动学习能力的影响。建立可行性更高、效率更高的的奖赏性康复训练及神经干预技术。  

Objectives of Study：

Exploring the impact of various types of reward-based rehabilitation training on the motivational rehabilitation of stroke patients. Investigating the effects of different types of reward-based rehabilitation training on the motor learning abilities of stroke patients. To establish more feasible and efficient reward-based rehabilitation training and neuro-intervention techniques.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

临床评估和综合影像学检查确诊为脑卒中的患者，符合 2014 年全国脑血管学术会议制定的诊断标准； 年龄 18- 80周岁，男女不限，右利手； 首次发病；病程≥6个月； Brunnstrom 分期Ⅲ~Ⅳ期； 可耐受半小时至一小时的评估过程； 受试者和其法定监护人理解、同意参加本研究并共同签署知情同意书.

Inclusion criteria

Patients diagnosed with stroke through clinical assessment and comprehensive imaging studies, in accordance with the diagnostic criteria set forth by the 2014 National Cerebrovascular Conference; Ages 18 to 80 years, regardless of gender, right-handed; First-time onset; disease duration of ≥6 months; Brunnstrom Stage III to IV; Capable of tolerating an assessment process lasting from half an hour to an hour; Participants and their legal guardians understand and agree to participate in this study, signing the informed consent form jointly.

排除标准：

有心肺疾病等严重系统性疾病不能耐受康复治疗者； 确诊精神病、重度抑郁症（有自杀倾向）或癫痫患者、有精神病或癫痫的家族史者； 存在尿毒症、心衰等严重系统性疾病； 严重的关节挛缩； 患有任何原因导致意识障碍； 听觉或视觉上有缺陷可能影响评估及治疗； 应用改变大脑皮质兴奋性的药物（抗癫痫药、镇静催眠类药物等）； 明显的疼痛、精神障碍

Exclusion criteria：

Patients with severe systemic diseases such as cardiopulmonary disorders that preclude tolerance of rehabilitation; Individuals diagnosed with psychosis, severe depression (with suicidal tendencies), or epilepsy, as well as those with a family history of mental illness or epilepsy; Those suffering from serious systemic conditions like uremia or heart failure; Severe joint contractures; Patients with consciousness disorders for any reason; Individuals with auditory or visual impairments that may affect evaluation and treatment.

Standard rehabilitation training and activities of daily living training. Functional training for the affected upper limb and hand, with 20 minutes of training content once daily for a total of 10 days. The training includes two tasks: pegboard and box and block tests. During the timed sessions of the pegboard training and box and block test, inform the patient that for each completion of a training session or test within the timed period, they will receive a 5 RMB reward.

常规康复训练，日常生活活动训练。患侧上肢和手的功能训练，20 分钟训练内容，每天一次，共计10天。训练内容包括两项任务：木插板和箱块试验。在木插板训练和箱块试验的计时训练时，告知患者：在计时训练过程中每完成一次训练或实验，都会获得抽奖机会（奖金分为1元，2元，5元，10元，15元五个级别）。并且计时训练中患者选择的任务难度越高，完成训练或实验后抽奖获得高额奖励的概率就越大（设置5种奖金的概率比例，使得每次获奖的平均概率只等于5元）

"Standard rehabilitation training, including daily living activities. Functional training for the affected upper limb and hand, consisting of a 20-minute session once a day for a total of 10 days. The training involves two tasks: pegboard and box and block test. During the timed sessions for the pegboard and box and block test, patients are informed that each time they complete a training session or test within the allotted time, they will earn a chance to draw a prize (with prize levels set at 1, 2, 5, 10, and 15 RMB). Moreover, the higher the difficulty of the task chosen by the patient during the timed training, the greater the probability of winning a higher monetary reward after completing the training or test (the probability ratios for the five prize levels are set so that the average probability of winning equates to 5 RMB)."
