Objectives of Study：

To evaluate the efficacy of allogeneic hematopoietic stem cell transplantation in the treatment of monogenic inflammatory bowel disease and analyze prognostic factors. To summarize the incidence of transplant related complications in the treatment of monogenic inflammatory bowel disease with allogeneic hematopoietic stem cell transplantation.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1 移植时年龄应大于2月且小于18岁； 2 单基因突变炎症性肠病诊断应基于以下标准： （1）具有炎症性肠病相关临床表现：腹泻、血便、腹痛和体重减轻等症状持续4周以上或6个月内反复发作2次以上，常合并发热、生长迟缓、营养不良、贫血等全身表现，可伴有肛周脓肿、皮赘、肛裂、肛瘘等肛周疾病； （2）内镜下检查：克罗恩病表现为病变呈节段性、非对称性、跳跃性分布，可见阿弗他溃疡、裂隙样溃疡、纵行溃疡，铺路石样肠粘膜、肠腔狭窄、肠壁僵硬；溃疡性结肠炎表现为黏膜呈颗粒状，充血、质脆易出血、血管纹理模糊或消失，弥漫性点状糜烂，浅溃疡或小溃疡； （3）组织病理学：克罗恩病组织病理学特点为全层肠壁淋巴细胞增生、非干酪样肉芽肿、局灶性隐窝结构异常、阿弗他溃疡、裂隙样溃疡，如为手术标本大体病理特点为肠管跳跃性病变、融合的线性溃疡、铺路石样改变、瘘管形成、肠腔狭窄；溃疡性结肠炎组织学表现为隐窝结构改变及炎性浸润； （4）通过二代测序或sanger测序发现炎症性肠病相关单基因突变，包括IL10、IL10R、FOXP3、CYBB、XIAP、LRBA、ZAP70、STAT3、IL2RA、CYBA、CD40LG等 同时满足（1）及（4），（2）或（3）满足至少一项； 3 接受异基因造血干细胞移植，供者包括同胞相合供者、非亲缘志愿者、非亲缘脐带血、单倍体供者。

Inclusion criteria

1 The age at transplant should be greater than 2 months and less than 18 years old. 2 The diagnosis of monogenic inflammatory bowel disease should be based on the following criteria: (1) Clinical manifestations: such as diarrhea, bloody stools, abdominal pain, and weight loss that persist for more than 4 weeks or recur more than 2 times within 6 months, often accompanied by systemic manifestations such as fever, growth retardation, malnutrition, anemia, and may be accompanied by perianal abscesses, dermatophytes, anal fissures, anal fistulas, and other perianal diseases. (2) Endoscopic examination: Crohn's disease is characterized by a segmental, asymmetric, and jumping distribution of lesions, with visible aphthous ulcers, fissure like ulcers, longitudinal ulcers, paving stone like intestinal mucosa, intestinal stenosis, and intestinal wall stiffness. Ulcerative colitis is characterized by granular mucosa, congestion, brittle texture, easy bleeding, blurred or disappeared vascular texture, diffuse punctate erosion, shallow or small ulcers. (3) Histopathology: The histopathological characteristics of Crohn's disease include full thickness intestinal wall lymphocyte proliferation, non-caseous granuloma, focal crypt structural abnormalities, aphthous ulcer, and fissure like ulcer. If the surgical specimen is used, the overall pathological characteristics are intestinal jumping lesions, fused linear ulcers, paving stone like changes, fistula formation, and intestinal stenosis. The histological manifestations of ulcerative colitis include changes in crypt structure and inflammatory infiltration. (4) Next-generation sequencing or Sanger sequencing showed mutations such as IL10, IL10R, FOXP3, CYBB, XIAP, LRBA, ZAP70, STAT3, IL2RA, CYBA, CD40LG. 3 Received allogeneic hematopoietic stem cell transplantation, including matched sibling donors, unrelated volunteers, unrelated umbilical cord blood, and haploid donors.

排除标准：

1 移植时存在严重脏器损害（如需要呼吸机辅助通气、依赖血管活性药物等）但仍接受挽救性移植的患儿； 2 二次移植的患儿。 3 高度怀疑为单基因炎症性肠病但未发现致病基因或存在相关致病基因不能排除为肠结核或其他感染性肠炎的患儿

Exclusion criteria：

1 Children who have severe organ damage during transplantation (such as requiring ventilator assisted ventilation, relying on vasoactive drugs, etc.) but still receive salvage transplantation. 2 Children undergoing secondary transplantation. 3 Children with highly suspected monogenic inflammatory bowel disease but no pathogenic genes found or the presence of related pathogenic genes cannot be ruled out as intestinal tuberculosis or other infectious enteritis.
