研究目的：

新疆医科大学第一附属医院与心韵恒安医疗科技(北京)有限公司共同完成“心血管疾病监测预警与智能管理平台”的功能完善和软件研发。在应用示范单位搭建“心血管疾病监测预警与智能管理平台”，在家庭、社区（基层医疗机构）、上级医疗机构进行应用测试，实现心电数据的长期监测，在家庭，基层医疗机构与上级医疗机构间构建心血管健康服务网络和随访平台；借助平台建立心血管疾病患者数据库；完成试验主办单位心血管疾病（主要是冠心病、心房颤动、心力衰竭等）患者的队列研究；建立起智能化心血管疾病患者监测与管理体系。  

Objectives of Study：

The development of the "Cardiovascular Disease Monitoring, Warning and Intelligent Management Platform" was jointly completed the functional improvement by the First Affiliated Hospital of Xinjiang Medical University and Xinyun Heng'an Medical Technology (Beijing) Co., Ltd. Establish a "cardiovascular disease monitoring, warning and intelligent management platform" in the application demonstration unit, conduct application testing in households, communities (grassroots medical institutions), and higher-level medical institutions, achieve long-term monitoring of electrocardiogram data, and build a cardiovascular health service network and follow-up platform between households, grassroots medical institutions, and higher-level medical institutions; Establishing a database of cardiovascular disease patients using the platform; Complete a cohort study of patients with cardiovascular diseases (mainly coronary heart disease, atrial fibrillation, heart failure, etc.); Establish an intelligent monitoring and management system for cardiovascular disease patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、年龄≥18岁。 2、房颤射频消融术后恢复窦性心律患者。 3、冠心病支架术后心电图ST段恢复至正常水平者。 4、初次诊断心衰的患者用口服药物治疗后。 5、在研究登记入组前，必须从合格患者处获得书面知情同意。

Inclusion criteria

1. Age ≥ 18 years old. 2. Patients with sinus rhythm restored after radiofrequency ablation of atrial fibrillation. 3. The ST segment of the electrocardiogram recovered to normal levels after coronary heart disease stent surgery. 4. Patients diagnosed with heart failure for the first time are treated with oral medication. 5. Prior to enrollment in the study, written informed consent must be obtained from qualified patients.

排除标准：

1、患者为妊娠或哺乳女性或计划妊娠的女性。 2、受试者已或需要植入起搏器、ICD、CRT或植入式血流动力学监测器。 3、患者存在严重肝肾功能不全或任何危及生命的重度疾病，预期寿命<1年。 4、患者有肾移植、心脏移植史。 5、恶性肿瘤患者。 6、不能独立配合完成研究的患者。

Exclusion criteria：

1. The patient is a pregnant or lactating woman or a woman planning to conceive. 2. The subject has or needs to be implanted with a pacemaker, ICD, CRT, or implantable hemodynamic monitor. 3. The patient has severe liver and kidney dysfunction or any life-threatening serious illness, with an expected life expectancy of less than 1 year. 4. The patient has a history of kidney or heart transplantation. 5. Patients with malignant tumors. 6. Patients who cannot cooperate independently to complete the study.
