研究目的：

(1) 拟明确不同时间实施神经阻滞镇痛对老年胸科手术患者，血流动力学的影响。 (2) 拟明确不同时间实施神经阻滞镇痛对老年胸科手术患者，围术期血管活性药物，以及麻醉药物使用剂量的影响。 (3) 拟明确不同时间实施神经阻滞镇痛对老年胸科手术患者，麻醉苏醒情况的影响。  

Objectives of Study：

(1) To determine the effect of nerve block and analgesia at different time on hemodynamics of elderly patients undergoing thoracic surgery. (2) To determine the effects of nerve block and analgesia at different times on elderly patients undergoing thoracic surgery, perioperative vasoactive drugs, and doses of anesthetic drugs. (3) To determine the effect of nerve block and analgesia at different times on the anesthesia recovery of elderly patients undergoing thoracic surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

行胸部择期手术及椎旁神经阻滞老年患者，年龄≥65岁，美国麻醉医师协会分级Ⅰ～III级，体重指数18-25 kg/m2。

Inclusion criteria

Patients who underwent elective thoracic surgery and paraverteal nerve block were ≥65 years old, American College of Anesthesiologists grade I to III, and had a body mass index of 18-25 kg/m2.

排除标准：

(1)术前发热：术前一天腋下体温＞37.5℃；(2)术前存在肺通气功能障碍者；(3)术前存在肺部感染、支气管哮喘或气道高反应者；(4)既往胸部科手术史；(5)心功能Ⅱ级以上者；(6)术前患有先心病、瓣膜病、冠心病、心律失常；(7)中枢神经系统疾病，脑卒中病史；(8)合并肝功能不全，肾功能不全；(9)术前放疗、化疗病史；(10)术前长期慢性疼痛史；(11)术前3个月内服用过止痛药物者。

Exclusion criteria：

(1) Preoperative fever: armpit temperature > 37.5℃ one day before surgery;(2) Patients with pulmonary ventilation dysfunction before operation;(3) Patients with pulmonary infection, bronchial asthma or airway hyperresponsiveness before operation;(4) Previous thoracic surgery history;(5) Patients with heart function above grade II;(6) preoperative congenital heart disease, valvular disease, coronary heart disease, arrhythmia;(7) Central nervous system disease, stroke history;(8) Combined with liver insufficiency and renal insufficiency;(9) History of preoperative radiotherapy and chemotherapy;(10) Long-term history of chronic pain before surgery;(11) Patients who had taken analgesic drugs within 3 months before surgery.
