比较全自动全芯活检针与全自动侧凹槽活检针在经皮肝脏穿刺活检中的有效性和安全性。  

Objectives of Study:

Compare the effectiveness and safety of a fully automatic core biopsy needle and a fully automatic side-notch biopsy needle in percutaneous liver biopsy

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

⑴ 原因不明的肝功能异常、肝硬化,以及需要明确有无肝纤维化或肝硬化的临床情况。 ⑵ 原因不明的肝肿大。 ⑶ 慢性乙型肝炎患者的抗病毒时机选择(评估肝纤维化或炎症坏死程度) 及疗效评估与监测,预后判断 ⑷ 考虑自身免疫性肝病,含自身免疫性肝炎、原发性胆汁性胆管炎、原发性硬化性胆管炎以及相互重叠的所谓重叠综合征。 ⑸ 考虑遗传代谢性肝病(豆状核变性、遗传性血色病、α1-抗胰蛋白酶缺乏症等),肝穿活检有助于诊断及治疗方案制定。 ⑹ 酒精性肝病与非酒精性脂肪性肝病的诊断及肝组织纤维化程度的确定。 ⑺ 肝移植患者术后,如考虑排斥反应或感染等并发症。 ⑻ 其他经多学科会诊讨论需要行肝脏穿刺活检的患者。

Inclusion criteria

Unexplained liver function abnormalities, cirrhosis, and the need to determine the presence of liver fibrosis or cirrhosis in clinical situations. Unexplained hepatomegaly (enlarged liver). Timing selection for antiviral treatment in patients with chronic hepatitis B (assessing the extent of liver fibrosis or inflammation) and evaluating treatment efficacy, monitoring, and prognosis. Consideration of autoimmune liver diseases, including autoimmune hepatitis, primary biliary cholangitis, primary sclerosing cholangitis, and overlapping syndromes. Consideration of genetic metabolic liver diseases (such as Wilson disease, hereditary hemochromatosis, alpha-1 antitrypsin deficiency, etc.), where liver biopsy assists in diagnosis and treatment planning. Diagnosis of alcoholic liver disease and non-alcoholic fatty liver disease, as well as determining the degree of liver tissue fibrosis. Post-liver transplant patients, when considering complications such as rejection reactions or infections. Other patients requiring liver biopsy as determined through multidisciplinary consultations.

排除标准:

⑴ 肝外梗阻性黄疸; ⑵ 有明显出血倾向(满足以下任意一项有出血倾向指凝血酶原时间(Prothrombin time, PT)延长>3 s和 (或) 血小板计数<50×109/L); ⑶ 昏迷或其他疾病不配合者; ⑷ 穿刺路径有感染病灶。

Exclusion criteria:

Extrahepatic obstructive jaundice. Significant bleeding tendency (meeting either of the following criteria: prolonged prothrombin time (PT) > 3 seconds and/or platelet count < 50×109/L). Comatose or non-cooperative patients with other illnesses. Presence of infectious lesions along the puncture path.

全芯活检组使用全自动全芯活检针,侧切槽活检组使用全自动侧凹槽活检针。均使用18G活检针及配套17G同轴针。穿刺针射程不低于20mm。记录活检样本长度、宽度和汇管区数量、样本碎片率、穿刺空针率等指标。同时记录每位患者最终的穿刺次数、操作时间。穿刺后即刻、1-2天、30天进行随访评估并发症情况。

Intervention:

The study compares the use of a fully automatic core biopsy needle in the fully core biopsy group and a fully automatic side-notch biopsy needle in the side-notch biopsy group. Both groups use an 18G biopsy needle with a matching 17G coaxial needle. The needle penetration depth is not less than 20mm. The following parameters are recorded: length and width of the biopsy sample, number of portal tracts, sample fragmentation rate, and rate of empty needle punctures. The number of punctures and the duration of the procedure are also recorded for each patient. Follow-up evaluations for complications are conducted immediately after the procedure, at 1-2 days, and at 30 days.
