研究目的：

本研究采用dTMS和rTMS对伴有下肢运动功能障碍的脑卒中患者进行干预，观察下肢功能量表及功能性评估和神经电生理评估指标变化并进行评价与分析。为有效改善卒中患者下肢运动功能的干预方法提供客观的试验依据，并进一步探讨dTMS影响卒中患者下肢运动功能的作用机制。  

Objectives of Study：

In this study, dTMS and rTMS were used to intervene in stroke patients with lower extremity motor dysfunction. The changes in the lower extremity function scale, functional assessment, and neuroelectrophysiological assessment indexes were observed and evaluated. To provide an objective experimental basis for effective intervention methods to improve the lower extremity motor function of stroke patients, and to further explore the mechanism of dTMS affecting the lower extremity motor function of stroke patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合2019年中华医学会神经病学会脑血管学组关于脑卒中的诊断标准，并经头颅 CT和/或MRI确诊确诊为脑出血或脑梗死； ②首次发病，病程≤6个月； ③脑卒中早期存在下肢运动障碍患者，运动诱发电位缺失或波幅小于健侧波幅的50%； ④年龄18岁≤年龄≤80岁； ⑤立位平衡≥1级，可辅助下完成至少步行10 m； ⑥神志清楚，可配合查体、无严重认知功能障碍的患者； ⑦患者及家属愿意接受rTMS或dTMS治疗，并签署知情同意书。

Inclusion criteria

① Meet the diagnostic criteria for stroke issued by the Chinese Society of Neurology, Chinese Medical Association, and be diagnosed as cerebral hemorrhage or cerebral infarction by head CT or MRI; ② The first onset, the course of disease ≤6 months; ③ Motor evoked potential was absent, or the amplitude was less than 50% of the amplitude of the healthy side in the early stage of stroke with lower extremity movement disorder; ④ 18 years old ≤ age ≤80 years old; ⑤ Standing balance ≥1 level, able to walk at least 10 m with assistance; ⑥ clear consciousness, able to cooperate with physical examination, and without severe cognitive impairment; ⑦ Patients and their family members were willing to receive rTMS or dTMS treatment and signed informed consent.

排除标准：

①头颅内置有金属异物者，带有心脏起搏器者，有耳蜗植入者； ②既往有癫痫史，有严重的心、肺、肝、肾等重要脏器功能衰竭者及有颅内高压症状者； ③严重的认知、交流障碍、情绪障碍、烦躁等不能配合者； ④参与本研究前3个月之内接受过rTMS或dTMS治疗； ⑤有颅骨缺陷等。

Exclusion criteria：

① metal foreign body in the skull, with a cardiac pacemaker, cochlear implantation; ② Patients with epilepsy history, severe heart, lung, liver, kidney, and other vital organ failure and intracranial hypertension symptoms; ③ Severe cognitive, communication disorders, emotional disorders, irritability, and other inability to cooperate; ④ Received rTMS or dTMS within three months before the study; ⑤ skull defects, etc.

我们采用随机数表法，以1：1的比例将患者随机分配到两个组，两组分别接受深部经颅磁刺激和重复经颅磁刺激。全部数据通过在线数据管理系统集中进行。

In a 1:1 ratio, patients were randomly assigned using a random number table to receive either deep transcranial magnetic stimulation or repetitive transcranial magnetic stimulation. All data were collected centrally through an online data management system.
