研究目的:

第一部分: 动脉钙化是糖尿病重要并发症,涉及多种致病因素,虽然糖尿病和动脉钙化之间的病理联系已经确立,但更深入理解糖尿病和动脉钙化之间潜在发病机制对确定新的分子治疗靶点至关重要。动脉钙化周围的脂肪、肌肉改变在动脉钙化的发生发展中具有重要作用。再次基础上,本实验拟收集糖尿病患者和血糖正常人群,利用CT从影像学上评价糖尿病动脉钙化的改变,探讨动脉钙化的影像标记物,为糖尿病动脉病变提供更充足的证据;并结合糖尿病血糖变化,深入解析解糖尿病动脉钙化病理生理机制,进一步探讨糖尿病动脉周围脂肪组织、肌肉改变与动脉钙化的相关性。 第二部分: 2型糖尿病(T2DM)、骨质疏松(OP)是老年人常见疾病。有报道指出,T2DM合并OP患者更易患肌少症。相关研究指出,OP是影响肌少症发生的重要因素,且经检测OP患者握力和骨量的变化趋势一致,提示OP患者或更易出现肌少症。亦有研究发现合并T2DM是影响肌少症发病的危险因素。本研究采用经CT定量测量糖尿病患者骨骼和肌肉情况,进一步探讨糖尿病、肌少症与骨质疏松的相关性。 第三部分: 骨质疏松症和心血管疾病是2型糖尿病中发病率高、致残率高、死亡率高的重要并发症,尤其严重影响老年人的生活质量和生存时间。动脉的异位矿化常伴有骨密度降低或骨转换紊乱。不同部位的骨密度值存在显著差异,且对于冠心病风险和冠心病识别能力等方面存在显著差异。在此基础上,本实验拟收集2型糖尿病患者,利用DECT、QCT从影像方面上定量评价椎体骨密度与冠脉钙化的关系,探讨椎体骨密度对于识别冠心病及评估冠心病风险的临床价值。  

Objectives of Study:

Part One: Arterial calcification is an important complication of diabetes and involves multiple pathogenic factors. Although the pathological link between diabetes and arterial calcification is well established, a better understanding of the underlying pathogenesis of diabetes and arterial calcification is essential to identify new molecular therapeutic targets. The changes of fat and muscle around arterial calcification play an important role in the occurrence and development of arterial calcification. On the basis of this, this study intends to collect diabetic patients and people with normal blood glucose, evaluate the changes of diabetic artery calcification from the imaging perspective by using CT, and explore the image markers of artery calcification, so as to provide more sufficient evidence for diabetic artery disease. The pathophysiological mechanism of arterial calcification in diabetic patients was analyzed in combination with the changes of blood glucose in diabetic patients, and the correlation between the changes of periarterial adipose tissue and muscle and arterial calcification was further discussed. Part Two: Type 2 diabetes mellitus (T2DM) and osteoporosis (OP) are common diseases in the elderly. It has been reported that T2DM patients with OP are more susceptible to sarcopenia. Relevant studies have pointed out that OP is an important factor affecting the occurrence of sarcopenia, and the change trend of grip strength and bone mass in OP patients is consistent, suggesting that OP patients may be more prone to sarcopenia. Some studies have also found that T2DM is a risk factor for the onset of sarcopenia. In this study, the bone and muscle of diabetic patients were quantitatively measured by CT to further explore the correlation between diabetes, sarcopenia and osteoporosis. Part Three: Osteoporosis and cardiovascular diseases are important complications of high incidence, disability and mortality in type 2 diabetes mellitus, especially affecting the quality of life and survival time of the elderly. Heterotopic arterial mineralization is often associated with decreased bone mineral density or bone turnover disorders. There were significant differences in BMD values at different sites, and there were significant differences in the risk of coronary heart disease and the ability to identify coronary heart disease. On this basis, this study intends to collect patients with type 2 diabetes mellitus, and use DECT and QCT to quantitatively evaluate the relationship between vertebral bone mineral density and coronary artery calcification from the perspective of imaging, so as to explore the clinical value of vertebral bone mineral density in the identification and risk assessment of coronary heart disease.

纳入标准:

① 年龄18周岁;② 按照WHO诊断标准明确诊断2型糖尿病;③ 基线检查时完成序列扫描模式进行胸部、腹部CT扫描;④ 能够理解本次研究目的及研究程序。

Inclusion criteria

① Age 18 years old; Type 2 diabetes was diagnosed according to WHO diagnostic criteria; ③ Chest and abdominal CT scans were performed in serial scan mode at baseline examination; ④ Be able to understand the purpose and procedure of this study.

排除标准:

排除标准:① 已接受或计划动脉手术;② 合并严重的肺(呼吸衰竭)、肝(ALT或AST正常值3倍及胆红素升高)、肾(GFR < 45ml/min/1.73m2或透析患者)功能不全的患者;③ 恶性肿瘤;④ 妊娠或哺乳期妇女或有生育计划者;⑤ 影响钙平衡状态和软组织钙化的伴随疾病(甲状旁腺功能亢进、结节病、淀粉样变性);⑥ 经研究者判断依从性差,无法按照要求完成研究。

Exclusion criteria:

① had received or planned arterial surgery; ② Patients with severe lung (respiratory failure), liver (ALT or AST 3 times normal and bilirubin elevated), kidney (GFR < 45ml/min/1.73m2 or dialysis patients) dysfunction; ③ Malignant tumor; Pregnant or lactating women or those who have a family plan; (5) concomitant diseases affecting calcium balance and soft tissue calcification (hyperparathyroidism, sarcoidosis, amyloidosis); ⑥ The researcher judged that the compliance was poor and the study could not be completed as required.
