研究目的：

在以全喉术后且无喉生存一定时间的患者为研究对象的前提下，探究喉-气管-食管-甲状腺多器官簇（可包含但不限于舌、下咽、食管、甲状旁腺）联合移植（以下简称为“喉气管移植”）的手术方式、围术期管理、功能康复及术后长期抗排异策略。从患者及其家属喉气管移植术前术后的心理测试结果、患者术前术后生活质量评估等结果对比此技术手段是否可改善患者及其家属的心理健康状态和生活质量；从基因组学、代谢组学、蛋白组学、呼吸道微生态、肠道微生态、体液及细胞免疫反应等多个维度出发，深入探究该手术对患者体内微环境的影响以及不同的基因及免疫状态对患者术后排异情况的影响，以期更精准地指导患者的围术期管理、术后长期抗排异策略以及术后长期随访计划的制定；从围术期肠内肠外营养支持、精准药物管理、预防颈部感染及其他并发症等精准护理角度出发，促进患者围手术期快速康复，早期进行功能锻炼，提高患者生活质量。  

Objectives of Study：

To explore the surgical procedure, perioperative management, functional rehabilitation and long-term anti-rejection strategy of combined laryngeal-tracheal-esophageal-thyroid multi-organ cluster transplantation (including but not limited to tongue, hypopharynx, esophagus and parathyroid gland) based on the patients who survived without larynx for a certain time after total laryngectomy. The psychological test results of patients and their families before and after laryngotracheal transplantation, and the quality of life evaluation results of patients before and after surgery were compared to compare whether this technology can improve the mental health and quality of life of patients and their families. From multiple dimensions such as genomics, metabolomics, proteomics, respiratory microecology, intestinal microecology, humoral and cellular immune response, the effects of the surgery on the patient's internal microenvironment and the effects of different gene and immune status on the patient's postoperative rejection were explored, In order to more accurately guide the perioperative management of patients, postoperative long-term anti-rejection strategy and postoperative long-term follow-up plan formulation; From the perspective of precise nursing such as perioperative enteral and parenteral nutrition support, precise drug management, prevention of neck infection and other complications, we can promote the rapid recovery of patients during the perioperative period, perform early functional exercise, and improve the quality of life of patients.

纳入标准：

入组患者需同时满足以下条件： （1）因各种病因（包括但不限于晚期喉癌、喉癌复发、下咽颈段食管癌、舌癌、创伤、晚期甲状腺癌术后复发等）导致全喉切除的患者； （2）年龄低于 75 岁，“无喉”生存时间达一年以上，对发声功能有迫切需求或为需高频用嗓的相关从业者； （3）全身基础情况良好或合并高血压、糖尿病、冠心病、甲状腺功能异常、肝肾功能异常等基础疾病经多科会诊评估后可行手术的患者； （4）愿意承担终生服用免疫抑制药物及所面临的风险或此前因其他病因（例如：已行其他器官移植术，某些风湿免疫类疾病等）已需终生服用免疫抑制药物的患者；

Inclusion criteria

Enrolled patients are required to fulfil the following conditions at the same time: (1) Patients who underwent total laryngectomy due to various causes (including but not limited to advanced laryngeal cancer, laryngeal cancer recurrence, hypopharyngeal and cervical esophageal cancer, tongue cancer, trauma, postoperative recurrence of advanced thyroid cancer, etc.); (2) Those who are under 75 years old, have lived "without laryngeal" for more than one year, and have an urgent need for voice function or are related practitioners who need to use throat frequently; (3) Patients with good general condition or combined with hypertension, diabetes, coronary heart disease, thyroid dysfunction, liver and kidney dysfunction and other basic diseases could be treated by surgery after multi-disciplinary consultation; (4) Willingness to take immunosuppressive drugs for life and the risks involved, or prior exposure to immunosuppressive drugs due to other causes (e.g.Other organ transplantation, some rheumatic diseases, etc.) patients who need life-long immunosuppressive drugs;

排除标准：

如果患者符合以下任一标准，则不入组: （1）年龄大于 75 岁，对发声功能等喉功能无迫切需求的患者； （2）全身情况较差，无法忍受喉气管移植手术及终生免疫抑制治疗的患者； （3）患者全喉切除术后目前处于带瘤生存状态（包括但不限于原位复发、远处转移、其他部位新发肿瘤）；

Exclusion criteria：

Patients were not enrolled if they met any of the following criteria: (1) Patients older than 75 years with no urgent need for laryngeal function such as voice function; (2) Patients with poor general condition who could not tolerate laryngotracheal transplantation and lifelong immunosuppressive therapy; (3) The patients were living with tumor after total laryngectomy (including but not limited to local recurrence, distant metastasis, and new tumors in other sites);
