研究目的：

本研究拟从复旦大学附属中山医院选择2型糖尿病患者，对其进行全面医学评估，由运动专家制定个体化运动处方，随后患者至各个具有运动康复师的社区运动中心接受专业的运动指导和监督，完成相应运动干预，随访6个月，观察患者糖尿病的改善情况，分析运动处方对糖尿病的安全性和有效性，探索治疗糖尿病的最佳运动种类、强度和时间，从而制定出适合中国糖尿病人群的体医融合方案。  

Objectives of Study：

The research plans to select type 2 diabetes patients from Zhongshan Hospital affiliated with Fudan University for a comprehensive medical assessment. A personalized exercise prescription will be developed by exercise experts. Subsequently, patients will attend community exercise centers with rehabilitation specialists for professional exercise guidance and supervision, completing the corresponding exercise interventions. A follow-up will be conducted for 6 months to observe the improvement of diabetes in patients, analyze the safety and effectiveness of the exercise prescription for diabetes, and explore the optimal types, intensity, and duration of exercise for treating diabetes. The goal is to formulate an integrated physical and medical solution suitable for the Chinese diabetic population.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 年龄：18 岁－80 岁； (2) 符合糖尿病诊断标准，且临床分型为 2 型糖尿病者； 根据《中国 2 型糖尿病防治指南 (2020 版) 》符合以下特征即可诊断为糖尿病： ① 存在典型糖尿病症状 (包括烦渴、多饮、多尿、多食、体重下降) ； ② 随机血糖 ≥ 11.1mmol/L 或空腹血糖 ≥7.0mmol/L 或 OGTT2h 血糖 ≥11.1mmol/L 或 HbA1c≥6.5%。 注：如果无①糖尿病典型症状者，需改日复查确认②。 (3)同意并签署知情同意书。

Inclusion criteria

(1) Age: 18 years - 80 years; (2) Meet the diagnostic criteria for diabetes, and clinically classified as type 2 diabetes. According to the "Chinese Guidelines for the Prevention and Treatment of Type 2 Diabetes (2020 edition)" , the diagnosis of diabetes can be confirmed if the following characteristics are met: ① Presence of typical symptoms of diabetes (including irritability, polydipsia, polyuria, polyphagia, weight loss); ② Random blood glucose ≥ 11.1mmol/L or fasting blood glucose ≥ 7.0mmol/L or OGTT 2-hour blood glucose ≥ 11.1mmol/L or HbA1c ≥ 6.5%. Note: If there are no typical symptoms of diabetes (①), a re-examination on another day is required to confirm (②). (3) Agree to and sign the informed consent form.

排除标准：

(1) 1 型糖尿病或其他特殊类型糖尿病； (2) 存在糖尿病急性并发症或血糖控制不良的糖尿病患者：HbA1c＞9.5%； (3) 合并有糖尿病严重慢性并发症者 (严重糖尿病肾病、严重神经病变、严重糖尿病视网膜病变、糖尿病足、下肢动脉闭塞) ； (4) 合并严重心脑血管疾病，包括心肌梗塞、充血性心衰者、严重心律失常、非稳定期的脑 血管意外、体位性低血压等； (5) 高血压控制不稳定者≥160/100mmHg； (6) 合并其他危急重症病史者；包括严重肝病、严重肾脏病、严重肺部疾病、红斑狼疮等； (7) 预期寿命不超过 5 年 (合并恶性肿瘤且病情控制欠佳) ； (8) 已经怀孕或者计划未来 12 周内怀孕的女性受试者； (9) 存在其他情况预计不能完成干预随访者； (10) 近 4 周内参加过其他临床试验； (11) 无法完成运动以及存在其他原因，研究者认为不适合参加本研究者。

Exclusion criteria：

(1) Type 1 diabetes or other special types of diabetes; (2) Patients with diabetes who have acute complications or poorly controlled blood sugar: HbA1c > 9.5%; (3) Those with severe chronic complications of diabetes (severe diabetic nephropathy, severe neuropathy, severe diabetic retinopathy, diabetic foot, lower limb arterial occlusion); (4) Those with severe cardiovascular and cerebrovascular diseases, including myocardial infarction, congestive heart failure, severe arrhythmias, non-stable cerebrovascular accidents, orthostatic hypotension, etc.; (5) Those with unstable blood pressure control ≥160/100mmHg; (6) Those with a history of other critical illnesses, including severe liver disease, severe kidney disease, severe lung disease, lupus, etc.; (7) Life expectancy not exceeding 5 years (complicated by malignant tumors and poor disease control); (8) Pregnant women or those planning to become pregnant within the next 12 weeks; (9) Participants with other conditions expected to prevent completion of intervention and follow-up; (10) Participation in other clinical trials in the past 4 weeks; (11) Inability to engage in physical activity and other reasons deemed unsuitable for participation by the investigator.
