研究目的：

（1）以成都市社区卫生服务中心糖尿病患者为对象，观察、分析现有糖尿病患者膳食结构和问题； （2）以慢性病系统软件为工具，探索低GI饮食在社区2型糖尿病患者血糖管理的应用； （3）以患者需求为导向整合分级诊疗模式，结合互联网医院—社区一体化慢性病管理模式，以期为社区2型糖尿病患者提供血糖综合管理优化策略，节约卫生资源，平台所获得的慢性病相关数据为省域内慢性病管理提供宝贵的原始资料。  

Objectives of Study：

1) to investigate the current dietary situation and problem among community type 2 diabetes patients in Chengdu; 2) Use chronic disease system software to explore the effect of low GI diet on the blood glucose among type 2 diabetes patients in the community; 3) Combined the needs of patients with the hierarchical diagnosis and treatment, the internet hospital and community integrated chronic disease management model is to provide the community type 2 diabetes patients with comprehensive blood glucose management optimization strategies, to save health resources, to collect chronic disease data on the platform and to provide valuable raw materials for chronic disease management in our province.

纳入标准：

1）符合糖尿病诊断标准的患者：经世界卫生组织(WHO)1999 年糖尿病诊断标准确诊的 18 周岁以上糖尿病患者(具有烦渴多饮、多尿、多食、不明原因的体重下降等典型糖尿病症状，且随机静脉血浆葡萄糖≥11.1 mmol/L 或空腹血浆血糖(FPG)≥7.0 mmol/L 或口服葡萄糖耐量试验(OGTT)2 h 血浆葡萄糖≥11.1 mmol/L)； 2)18岁≤年龄≤75岁； 3)糖尿病病程≥3 个月 4）能使用智能手机； 5）自愿受试，签署知情同意书；

Inclusion criteria

1) Patients who meet the diagnostic criteria for diabetes: diabetes patients over 18 years of age diagnosed according to the World Health Organization (WHO) diagnostic criteria for diabetes in 1999 (with typical diabetes symptoms such as polydipsia, polyuria, polyphagia, unexplained weight loss, and random intravenous plasma glucose ≥ 11.1 mmol/L or fasting plasma glucose (FPG) ≥ 7.0 mmol/L or oral glucose tolerance test (OGTT) 2h plasma glucose ≥ 11.1 mmol/L); 2) 18 years old ≤ Age ≤ 75 years old; 3) the duration of diabetes ≥ 3 months 4) Able to use smartphones; 5) Voluntarily participate in the trial and sign an informed consent form;

排除标准：

1）酮症酸中毒、高渗性非酮症糖尿病昏迷等患者； 2）其他严重糖尿病并发症； 3）明显肝肾疾病的症状和体征； 4）急性心脑血管病患者、急慢性胰腺炎及胰腺功能不全患者 5）维生素 B12、叶酸和铁缺乏的患者 6）严重外伤或接受手术者；严重感染者； 7）严重的无意识性低血糖病史者； 8）严重高血压患者（收缩压（SBP）＞160 mmHg，舒张压（DBP）＞95 mmHg）； 9）精神疾病史及家族史者； 10）1型糖尿病、妊娠期糖尿病； 11）胃肠道功能紊乱和相关疾病；

Exclusion criteria：

1) Patients with ketoacidosis, hypertonic non ketotic diabetes coma, etc; 2) Other serious complications of diabetes; 3) Symptoms and signs of obvious liver and kidney diseases; 4) Patients with acute cardiovascular and cerebrovascular disease, acute and chronic pancreatitis, and pancreatic insufficiency 5) Patients with vitamin B12, folate, and iron deficiency 6) Severe trauma or surgery; Severely infected individuals; 7) Individuals with a history of severe unintentional hypoglycemia; 8) Patients with severe hypertension (systolic blood pressure (SBP)>160 mmHg, diastolic blood pressure (DBP)>95 mmHg); 9) Individuals with a history of mental illness and family history; 10) Type 1 diabetes, gestational diabetes; 11) Gastrointestinal dysfunction and related diseases;
