研究目的:

1)老年慢阻肺病流行病调查及运动干预研究具有高度的复杂性,涵盖了体育学、医学(流行病学)、心理学、社会学等多个学科领域,学科知识交叉融合,单一学科的知识难以解决涉及健康各层面问题,因此,多学科多手段的贯通是该领域研究的必由之路。 (2)对老年慢阻肺人群进行横向调查和监测, 以记录慢阻肺病流行病学害处, 了解慢阻肺病流行病学特征, 并确定个人和环境风险因素,构建“头低位强腹式呼吸训练”运动干预模式和实施路径。  

Objectives of Study:

(1) Research on epidemiological surveys and exercise interventions for the elderly with chronic obstructive pulmonary disease is highly complex, covering a number of disciplines such as kinesiology, medicine (epidemiology), psychology, sociology, etc. The knowledge of the disciplines is cross-fertilized, and it is difficult to solve problems involving all aspects of health with the knowledge of a single discipline.(2) A cross-sectional survey and monitoring of the elderly population with chronic obstructive pulmonary disease (COPD) was conducted to document the epidemiological risks of COPD, to understand the epidemiological characteristics of COPD and to identify personal and environmental risk factors, and to develop an exercise intervention model and implementation pathway for "head-down strong abdominal breathing training".

纳入标准:

1.根据全球阻塞性肺疾病启动(GOLD)标准(1s用力呼气量(FEV1)/用力肺活量(FVC)<70%)确诊为COPD 2.同时处于GOLDⅠ-Ⅳ阶段 3.能够理解学习指令 4.MMSE评分在23至27之间(最高总分是30分,23-27分表示轻度认知障碍(MCI))

Inclusion criteria

(1) Diagnosis of COPD according to the Global Obstructive Lung Disease Initiation (GOLD) criteria (exertional expiratory volume in 1s (FEV1)/exertional spirometry (FVC) <70%), while in GOLD stagesⅠ-Ⅳ; (2) MMSE score between 23 and 27 (maximum total score is 30, with 23-27 indicating MCI;

排除标准:

1.由于认知障碍而无法遵循指示或完成我们研究的问卷 2.中风史、高危心肺疾病或骨科疾病,如危重主动脉瓣狭窄 3.心肌梗死后早期或下肢截肢 4.难以完成心肺运动试验(CPET)或6MWT 5.诊断为肺癌或胸腹手术史 6.体重指数(BMI)为≥30 7.视力或听力缺陷 8.接受可能导致认知障碍的药物 9.在过去12个月内完成了肺部康复,或目前正在参加锻炼计划 10.严重认知障碍/痴呆(蒙特利尔认知评估评分<17分)或其他诊断的神经精神症状

Exclusion criteria:

(1) Inability to follow RMT instructions or complete questionnaires for our study due to cognitive impairment; (2)history of stroke, high-risk cardiopulmonary disease or orthopaedic disease within the previous 4 weeks; (3) Diagnosis of lung cancer or history of thoracoabdominal surgery; (4) A body mass index (BMI) of ≥30; (5) pleural disease or thoracic deformity (6) visual or hearing impairment (7) receiving medication that may cause cognitive impairment (8) completed pulmonary rehabilitation within the last 12 months or currently participating in an exercise programme (9) Severe cognitive impairment/dementia (Montreal Cognitive Assessment score <17) or other diagnosed neuropsychiatric symptoms.

选择了健身气功八段锦中对呼吸功能有调节作用的两套动作: "两手托天管三焦,左右开弓似射鹰"。每节课40分钟,包括运动开始时5分钟的热身适应,30分钟的头下强腹式呼吸练习和5分钟的拉伸练习放松,每周3次,干预周期为12周。

The two sets of movements in the Eight Duan Jin of Fitness Qigong, which have a regulatory effect on the respiratory function, were selected: "Two hands resting on the sky to manage the three jiao, and left and right opening the bow like a shooting eagle". Each session is 40 min and consists of a 5 min warm-up adaptation at the beginning of the exercise, 30 min of head-down strong abdominal breathing exercises and 5 min of stretching exercises to relax, three times a week for a 12-week intervention cycle.

实验前患者将有一周的时间集中了解和学习低头强腹运动,以保证在正式实验中,本组的参与者能规范地完成相关动作。实验过程中,按照实验要求,由易到难进行低头强腹式呼吸,并要求被试者排空大便,至少在饭后1h进行低头强腹式呼吸,并在一次训练中完成。干预强度为每周三次,每次30分钟,包括运动开始时5分钟的热身和适应,20分钟的低头强腹式呼吸,5分钟的拉伸和放松,呼吸频率为35次/分钟(节拍器提示),在倾角为-30°的倒立两用运动机(专利号201821510570.5)上进行低头训练。干预期为12周。本研究还将使用这种倒置的颠倒两用运动机进行头朝下和强腹式呼吸练习。

Intervention:

Patients will have a week before the experiment to focus on understanding and learning the head down and strong abdominal exercise to ensure that the participants in the group can perform the relevant movements in a standard way during the formal experiment. During the experiment, head down and strong abdominal breathing will be performed in accordance with the experimental requirements, from easy to difficult, and the participant will be asked to empty their bowels and to perform the head down and strong abdominal breathing at least 1 h after a meal, and to complete the training in one session. The intensity of the intervention was 30 min three times per week, including 5 min of warm-up and acclimatization at the beginning of the exercise, 20 min of head down and strong abdominal breathing, 5 min of stretching and relaxation, with a respiratory rate of 35 breaths/min (metronome cue), and head-down training on an inverted upside-down dual-purpose exercise machine (Patent No. 201821510570.5) at an inclination angle of -30°. The intervention period was 12 weeks. This study will also use this inverted upside-down dual-purpose exercise machine to perform head down and strong abdominal breathing exercises.
