研究目的：

显著门静脉高压限制了肝癌患者治疗方式的选择，严重影响患者的预后。 合并门静脉高压的肝癌患者具有显著异质性，治疗决策仍存在较大争议。本项研究的目的是对比肝切除和经肝动脉灌注化疗栓塞对合并门静脉高压肝癌患者的安全性及生存获益情况。  

Objectives of Study：

Clinically significant portal hypertension (CSPH) limits the therapeutic options for hepatocellular carcinoma (HCC), which is closely associated with patient prognosis. HCC patients with CPSH are heterogeneous and treatment allocation remains controversial. The aim of this study was to compare the safety and survival benefit of liver resection (LR) and transarterial chemoembolisation (TACE) in these populations.

纳入标准：

本研究纳入根据美国肝病研究协会/欧洲肝脏研究协会指南确诊为肝癌并行肝切除术或经肝动脉灌注化疗栓塞术的患者。

Inclusion criteria

This study included patients who were diagnosed with HCC according to the American Association for the Study of the Liver Disease/European Association for the Study of the Liver Guidelines and who received conventional LR or TACE.

排除标准：

患者如果存在下述标准，将被排除：（1）合并三/四型门静脉癌栓，肝动脉癌栓，胆管癌栓或者肝静脉癌栓。（2）存在肝外转移；（3）前期经过治疗；（4）东部肿瘤合作组表现状态大于1分；（5）弥散性肿瘤；（6）合并其他肿瘤或严重的心、脑、肾功能不全；（7）不存在显著门静脉高压；（8）缺少基线信息

Exclusion criteria：

Patients were?excluded?if?at?least?one?of?the?following?pre-defined?criteria?was met: (1) presence of types III/IV portal vein tumour thrombosis (PVTT), hepatic artery, biliary duct or inferior vena cava invasion; (2) presence of extrahepatic spread (EHS); (3) underwent previous treatments; (4) Eastern Cooperative Oncology Group Performance Status (ECOG-PS) >1; (5) diffuse tumor nodules; (6) combined with other tumours or severe cardiac, cerebral, and renal insufficiency; (7) non-CSPH, and (8) absence of baseline information.
