研究目的:

第一阶段:采集CTEPH 患者影像学资料 (CTPA/DSA 造影),使用3DSlicer、Python 等软件识别并标记体-肺侧枝,分割图像提取影像组学特征(数据1)。 PEA 术中通过体外循环精确测量的真实体-肺分流量数据(数据2)。记录术后患者再灌注肺水肿情况(数据3)。使用人功智能 (AI) 技术对上述资料进行匹配及深度学习并,结合相关性分析等统计方法建立可靠的体-肺侧枝分流量及术后再灌注肺水肿高危预测模型。 第二阶段:通过建立的风险预测模型识别出术后再灌注肺水肿高危人群后随机对照分组,试验组术前进行体-肺侧枝栓塞,比较两组间术后再灌注肺水肿等临床事件发生率及程度。通过数据分析明确对术后再灌注肺水肿高危患者行术前体-肺侧枝栓塞能否降低其发生率和严重程度。  

Objectives of Study:

Stage 1: Imaging data (CTPA/DSA imaging) of CTEPH patients were collected, and software such as 3DSlicer and Python was used to identify and label the somatopleulopulmonary collateral branches, and image segmentation was performed to extract the imaging features (Data 1). Data on true-lung shunt volume accurately measured by cardiopulmonary bypass during PEA surgery (Data 2). Postoperative reperfusion pulmonary edema was recorded (data 3). Human power intelligence (AI) technology was used to match the above data and deep learning, and a reliable prediction model of body-lung collateral shunt volume and postoperative reperfusion pulmonary edema risk was established in combination with statistical methods such as correlation analysis. The second stage: The high-risk groups of postoperative reperfusion pulmonary edema were identified by the established risk prediction model and then randomly divided into control groups. The experimental group underwent somatic and pulmonary collateral embolization before surgery, and the incidence and degree of clinical events such as postoperative reperfusion pulmonary edema were compared between the two groups. Data analysis was used to determine whether preoperative systemic and lateral pulmonary embolization could reduce the incidence and severity of postoperative reperfusion pulmonary edema in high-risk patients.

① 诊断为慢性血栓栓塞性肺动脉高压、慢性肺血栓栓塞症或肺动脉占位
② 年龄10-85岁
③ 拟行肺动脉内膜剥脱术
④ 模型预测为再灌注肺水肿高风险
⑤ 取得知情同意

Inclusion criteria

① The diagnosis was chronic thromboembolic pulmonary hypertension, chronic pulmonary thromboembolism, or pulmonary artery occupying
② Age 10-85 years old
③ Pulmonary endarterectomy was performed
④ The model predicted a high risk of reperfusion pulmonary edema
⑤ Obtain informed consent

排除标准:

① 未取得知情同意
② 存在严重肺实质疾病,如重度慢阻肺、重度肺部感染、巨大肺部空洞等
③ 存在肝功能或肾功能衰竭
④ 模型预测为再灌注肺水肿低风险
⑤ 因各种原因无法行支气管动脉造影或栓塞

Exclusion criteria:

① Without informed consent
② There are serious pulmonary parenchymal diseases, such as severe COPD, severe pulmonary infection, and giant pulmonary cavities
③The presence of liver or renal failure
④ The model predicted a low risk of reperfusion pulmonary edema
⑤ Bronchial arteriography or embolization could not be performed for various reasons
