研究目的：

1. 主要目的：对比常规临床管理模式，评估“健康接力”模式对抑郁症患者的治疗依从性、治疗结局效果（1 个月、3 个月、6 个月时间节点）差异； 2. 次要目的：1）对比常规临床管理模式，评估“健康接力”模式对抑郁症患者自 我感知的社会支持（perceived social support）、自我效能(self-efficacy)；2）初步评估“健康接力”模式在抑郁症患 者中的接受度、可行性及其社会行为心理作用机制。  

Objectives of Study：

1.Main objective: to compare the conventional clinical management model with the "Pay it Forward" model, and to evaluate the difference of treatment compliance and treatment outcome (1 month, 3 months, 6 months) in patients with depression. 2.Secondary objectives: 1) to compare the differences of perceived social support and self-efficacy between conventional clinical management model and "Pay it Forward" model in patients with depression; 2) to evaluate the acceptability, feasibility and social behavior psychological mechanism of the "Pay it Forward" model in patients with depression.

纳入标准：

i) 18 岁及以上成年人； ii) 有继续在本地居住半年或以上的打算； iii) 经临床评 估为中重型抑郁症（MDD）患者且有抗抑郁药物治疗指征；iv) 愿意参与本研究并提供联系方式以供科研团队干预随访

Inclusion criteria

I) adults aged 18 and above; ii) have plans to continue to live locally for half a year or more; iii) diagnosed with major depression disorder (MDD) and have indications for antidepressant treatment; iv) is willing to participate in this study and provide contact information for follow-up intervention by the research team.

排除标准：

i) 有其他不稳定性急慢性躯体疾病（如心脑血管、肿瘤等疾病）亟需住院或其他药物治疗者；iv) 共病其他精神疾病者（如物质依赖等）；v) 不愿提供联系方式者。

Exclusion criteria：

i) patients with other unstable acute and chronic somatic diseases (such as cardiovascular and cerebrovascular diseases, tumors, etc.) who are in urgent need of hospitalization or other drug treatment; iv) patients with other mental disorders (such as substance dependence, etc.); v) those who are unwilling to provide contact information.

由专职科研人员告知参与患者，此前有其它抑郁症患者因为感同身受和关心，进行了捐款以支付该参与者的第一次抗抑郁药物治疗的费用，同时还通过明信片/微信信息分享了他的个人故事并鼓励参与患者积极配合医生接受治疗并按时随 访，若患者有意愿接受药物治疗并将善意传递下去，我们将进一步询问其在未来的 6 个 月时间是否也愿意通过微信分享自己的个人经历或捐款以支持鼓励更多患者积极治疗。 随后科研人员将参与患者转入同一临床医师进行临床标准化管理（过程与对照组相同）。 我们将在现场提供募捐箱或在线捐赠二维码。无论是捐赠意愿还是捐赠金额均不会影响 他们接受捐赠享受第一次免费药物治疗，也不影响任何其它临床医疗服务。另外，我们 将每隔 1 个月反馈给参与者其他患者分享的故事/捐款对他人产生的积极作用，并定期 在项目组微信公众号公布募捐额度及善款使用详情。

干预措施代码：

Intervention：

The researcher informed the subject that other patients with depression had donated money to pay for the participant's first antidepressant treatment because of empathy and concern. At the same time, the kind patient also shared his personal story through postcard / Wechat information and encouraged the subject to actively cooperate with the doctor for treatment and follow-up on time. If the subject is willing to receive medication and pass on his kindness, we will further ask whether he or she is also willing to share his or her personal experience or donation through Wechat in the next 6 months to support and encourage more patients to take active treatment. The researchers then transferred the patients to the same clinician for clinical standardized management (the process is the same as the control group). We will provide a donation box or online donation QR code on site. Neither the willingness to donate nor the amount of donation will affect their acceptance of donation to enjoy the first free drug treatment, nor will it affect any other clinical medical services. In addition, we will feedback the positive effects of the stories / donations shared with other patients to the participants every other month, and regularly announce the fund-raising amount and the details of the use of the donation on the project group's official Wechat account.
