研究目的：

探讨右美托咪定，氟比洛酚酯联合超声引导下双侧肋缘下TAPB多模式预镇痛方案对行腹腔镜结直肠手术老年患者POGD的发生率及肠道菌群丰度的影响，为老年患者术后胃肠道功能快速恢复提供新思路，促进老年患者术后ERAS。  

Objectives of Study：

To investigate the effects of dexmedetomidine, flurbiprofenaxetil combined with ultrasound-guided bilateral subcostal TAPB multimodal preanalgesia regimen on the incidence of POGD and the abundance of intestinal flora in elderly patients undergoing laparoscopic colorectal surgery, so as to provide new ideas for the rapid recovery of gastrointestinal function and promote postoperative ERAS in elderly patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥65岁； （2）ASA分级Ⅰ-Ⅲ级； （3）择期全身麻醉下行腹腔镜结直肠手术。

Inclusion criteria

(1) age ≥65 years old; (2) ASA Ⅰ-Ⅲ; (3) Selective laparoscopic colorectal surgery under general anesthesia.

排除标准：

（1）既往有胃肠运动障碍、腹部手术史； （2）重要脏器功能不全，如：严重肝肾功能障碍、二度或三度房室传导阻滞、慢速心律失常(基线心率低于50次/分钟)； （3）精神障碍、有气道困难或延迟拔管史； （4）滥用阿片类药物、术前化疗史； （5）相关麻醉药物过敏； （6）术中大量出血(出血量>800 mL)； （7）ASA分级Ⅳ-Ⅴ级、急诊手术、手术中转开腹。

Exclusion criteria：

(1) Previous history of gastrointestinal motility disorders and abdominal surgery; (2) vital organ dysfunction, such as severe liver and kidney dysfunction, second or third degree atrioventricular block, or tachyarrhythmia (baseline heart rate < 50 beats/min); (3) history of mental disorder, airway difficulty or delayed extubation; (4) history of opioid abuse and preoperative chemotherapy; (5) allergy to related anesthetic drugs; (6) Massive intraoperative bleeding (bleeding volume > 800 mL); (7) ASA Ⅳ-Ⅴ, emergency surgery, and conversion to open surgery.

术中给予0.3 μg/kg/h右美托咪定持续泵注至手术结束前30分钟，术毕前10min给予氟比洛酚酯1mg/kg。手术结束之后给予超声引导下双侧TAPB （0.375%罗哌卡因，每侧15ml）。

Intraoperative administration of 0.3 μg/kg/h dexmedetomidine should be continuously pumped until 30 minutes before the end of surgery, and flurbiprofen axetil 1mg/kg was given 10min before the end of the operation. After the operation, ultrasound-guided bilateral TAPB was performed (0.375% ropivacaine, unilateral 15ml).

麻醉诱导前10min给予右美托咪定0.5μg/kg，术中维持 0.3μg/kg/h右美托咪定持续泵注至手术结束前30分钟，同时在麻醉诱导前给予超声引导下双侧TAPB（0.375%罗哌卡因，每侧15ml）。手术切皮前5min给予氟比洛酚酯1mg/kg ，行多模式预镇痛。

Intervention：

Dexmedetomidine 0.5μg/kg was given 10min before anesthesia induction, and Intraoperative maintenance of 0.3 μ G/kg/h dexmedetomidine should be continuously pumped until 30 minutes before the end of surgery. At the same time, ultrasound-guided bilateral TAPB (0.375% ropivacaine, unilateral 15ml) was given before anesthesia induction. 5 minutes before skin incision, flurbiprofen axetil 1mg/kg was given for multimodal preanalgesia.
