研究目的：

本研究旨在利用磁共振成像图像和其他潜在的临床因素构建深度学习模型，术前预测肝动脉化疗栓塞治疗肝细胞癌的早期疗效，并初步探讨模型预测过程相关的生物学基础。  

Objectives of Study：

This study aimed to construct a deep learning model using magnetic resonance imaging images and other potential clinical factors to predict the early efficacy of transcatheter arterial chemoembolization for hepatocellular carcinoma and to explore the biological basis of the model prediction process.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)在浙江大学丽水医院 (2016年4月至2022年12月)，浙江大学医学院附属第一医院( 2018年1月至2021年12月)和杭州市第一人民医院 ( 2017年1月至2021年12月) 首次接受TACE治疗的肝癌患者；经影像学检查确诊为肝癌，若结节在医学图像中表现不典型，则需结合活检结果确诊； (2) TACE术前及术后行增强磁共振成像(CEMRI)检查； (3)完整的临床信息 (即人口统计学，肝功能检测等)。

Inclusion criteria

(a) Patients with hepatocellular carcinoma who underwent initial transarterial chemoembolization (TACE) in Lishui Hospital of Zhejiang University (April 1, 2016, to December 31, 2022), the First Affiliated Hospital, Zhejiang University School of Medicine (January 1, 2018, to December 31, 2021), and the Hangzhou First People’s Hospital (January 1, 2017, to December 31, 2021); HCC was confirmed by imaging examinations evaluation. Biopsy was carried out if the nodule did not indicate typical features in medical images. (b) Contrast-enhanced magnetic resonance imaging (CE-MRI) was performed before and after TACE; (c) Relevant clinical information was available (i.e. demographics, preoperative hepatitis, serum alpha-fetoprotein (AFP) levels, and liver function tests).

排除标准：

(1)未成年患者； (2)随访期间同步治疗，如手术切除、全身化疗等； (3)合并其他恶性肿瘤，术后随访3个月以内。 (4)感兴趣区(region of interest，ROI)过小或MRI图像质量差的患者。

Exclusion criteria：

(a) Underage patients; (b) Synchronous therapies during follow-up time, such as resection, and systemic chemotherapy; (c) With other malignancies and follow-up for less than 3 months after the operation. (d) Very small region of interest (ROI) or patients with poor MRI image quality.

组别：

所有纳入患者均接受TACE治疗，包括常规TACE和载药微球TACE。由2名经验丰富的腹部放射科医师根据术后CEMRI 确定客观反应。根据改良的实体肿瘤疗效评价标准(modified Response Evaluation Criteria in Solid Tumor，mRECIST )进行TACE疗效评价。简而言之，TACE的治疗反应分为四个等级：( a )完全缓解(complete response，CR)：病灶完全消失；(b)部分缓解(partial response，PR)：有效靶病灶直径之和(动脉期强化明显)至少减少30 %；(c)疾病进展(progressive disease，PD)：至少有20 %的存活(增强)靶病灶直径之和增大；(d)疾病稳定(stable disease，SD)：无PR或PD。根据mRECIST标准，将CR和PR患者归为客观缓解组，PD和SD患者归为非客观化缓解组。

Group：

All included patients were treated with TACE, including conventional TACE (hereafter, cTACE) and drug-eluting bead TACE (hereafter, DEB-TACE), as described in Appendix S2. The objective response was determined by two experienced abdominal radiologists according to the post-operative contrast-enhanced magnetic resonance imaging (CE-MRI) within 3 months. TACE response was performed according to the modified Response Evaluation Criteria in Solid Tumors (mRECIST) [15] criterion. In brief, the therapeutic response of TACE was classified into four grades, including complete response (CR), partial response (PR), progressive disease (PD), and stable disease (SD). CR and PR patients were categorized as the objective response (OR) group, while PD and SD patients as non-objective response (NOR) group. The examples of MR images of OR and NOR in Patients with HCC were obtained according to the mRECIST standard.
