研究目的:

观察超声引导不同入路喉上神经阻滞对全麻患者术后咽痛的影响,评价其安全性和有效性,为该技术的临床应用提供理论依据。  

Objectives of Study:

To observe the influence of ultrasound-guided superior laryngeal nerve block on postoperative pharyngodynia in patients with general anesthesia, evaluate its safety and effectiveness, and provide theoretical basis for the clinical application of this technology.

药物成份或治疗方案详述:

纳入标准:

① 进行气管插管的全麻患者; ② 18 ~ 60岁的男性或女性; ③ ASAⅠ或Ⅱ级; ④体重指数(BMI)18~28 kg/m2 ⑤手术时长在2h内的患者

Inclusion criteria

① General anesthesia patients undergoing endotracheal intubation; ② Male or female aged 18-60; ③ ASAⅠ or Ⅱ level; ④ Body mass index (BMI) 18-28 kg/m2 ⑤ Patients with operation duration within 2 hours

排除标准:

①长期有吸烟史的患者; ②术前长期咽喉部不适及慢性咽喉炎的患者; ③存在插管困难、多次插管的患者; ④口咽及颈部手术的患者; ⑤长期服用阿片类药物、解热镇痛药或激素治疗的患者; ⑥对局部麻醉药有过敏的患者; ⑦无法理解或配合疼痛评分的患者。

Exclusion criteria:

① Patients with a long history of smoking; ② Patients with long-term pharyngeal discomfort and chronic pharyngitis before surgery; ③ Patients with intubation difficulties or multiple intubations; ④ Patients undergoing oropharyngeal and neck surgery; ⑤ Patients taking opioids, antipyretic analgesics or hormone therapy for a long time; ⑥ Patients who are allergic to local anesthetics; ⑦ Patients who are unable to understand or coordinate pain scores.

将探头放置在甲状腺软骨(TC)的横切面上。调整探头的中线,直到甲状软骨在屏幕中间清晰地显示出来。然后移动探头寻找甲状腺切迹(TC断开的部分)。并通过类似“拱门”的结构,识别甲状舌骨膜,使用神经阻滞针在甲状舌骨膜中线行超声引导平面外注射。回抽无血后,注射0.5%罗哌卡因2ml,注射时阻力较小。药物会将会厌前间隙推开,并迅速扩散到侧间隙。

Intervention:

The probe will be placed on the transverse section of the thyroid cartilage (TC). Adjust the center line of the probe until the thyroid cartilage is clearly displayed in the middle of the screen. The probe is then moved to look for the thyroid notch (the part where the TC is disconnected). The thyrohyosteal was identified by the structure similar to the "arch", and ultrasound guided out-of-plane injection was performed in the median line of the thyrohyosteal using nerve blocking needles. After no blood was drawn back, 2ml 0.5% ropivacaine was injected with less resistance. The drug will push away the preepiglottic space and quickly spread to the lateral space.

Intervention:

The probe was placed on the sagittal surface of the submandibular region. The hyoid bone was ultrasonically identified as a hyperechoic, thyroid (hypoechoic) structure. The thyrohyosteal (hyperechoic) is located between these two structures. An ultrasus-guided, extra-plane injection of 2ml 0.5% ropivacaine is administered on the thyrohyosteal with the tip of a nerve block needle along the upper border of the thyroid cartilage (no blood drawn back). The same drug will push away the preepiglotic space. The contralateral superior laryngeal nerve was blocked in the same way.

患者取仰卧位,在颈外侧触及舌骨角及甲状软骨上角,在其连线中点用穿刺针垂直皮肤穿刺可以阻滞喉上神经内支,当刺破甲状舌骨韧带,稍有突破感,回吸无血后,注入0.5%罗哌卡因2ml,用相同方法阻滞对侧。

The patient was placed in a supine position and touched the hyoid horn and the superior horn of thyroid cartilage in the lateral neck, and the internal branch of the superior laryngeal nerve could be blocked by vertical skin puncture with a puncture needle at the midpoint of the connection. When the thyrohyoid ligament was punctured, a slight breakthrough was felt, and 2ml of 0.5% ropivacaine was injected after suction without blood, and the opposite side was blocked by the same method.
