明确早期心脏康复对心血管疾病患者长期预后的影响。  

Objectives of Study：

To investigate the long-term prognosis of cardiac rehabilitation in patients with cardiovascular diseases.

药物成份或治疗方案详述：

纳入标准：

研究期间，在心血管医学部及康复医学科的住院患者，主要诊断为心血管疾病的患者，愿意签署心脏康复知情同意书。

Inclusion criteria

In-hospital patients in department of cardiology and department of rehabilitation medicine, willing to sign the inform consents.

排除标准：

（1）住院期间持续存在运动禁忌症；（2）在院期间存在意识障碍无法配合评估的患者。运动禁忌症包括：(1)不稳定型心绞痛或急性心肌梗死后病情不稳定;(2)出现新的心电图心肌缺血改变; (3)危重抢救患者在严密监护下; (4)静息时 SBP＞180mHg 或 DBP＞110 mmhg (5)血液动力学不稳定或低血压，包括血压的异常反应(直立或运动时血压明显并有症状) (6)严重心律失常(静息时心率＞120次/min、没有控制的房颤或室上性心动过速室早＞15/100次、高度房室传导阻滞); (7)没有控制的心衰或心源性休克; (8)严重合并症，包括体温超过 38℃、急性心肌炎或心包炎、未控制的糖尿病或黏液性水肿、血栓或栓塞; (9)动脉瘤或严重瓣膜疾病或主动脉狭窄; (10)发绀型先天性心脏病;(11)梗阻性肥厚型心肌病; (12)严重肺动脉高压； (13)急性全身性疾病或肝肾疾病 (14)洋地黄或其他药物中毒; (15)手术切口异常; (16)严重贫血或电解质乱 (17)运动诱发支气管痉挛 (18)明显精神紧张或患者不理解或不配合进行康复治疗。

Exclusion criteria：

(1) Continuous contraindications to exercise during hospitalization; (2) Patients with impaired consciousness during hospitalization who are unable to cooperate with assessment. Contraindications to exercise include: (1) Unstable angina or unstable condition after acute myocardial infarction; (2) New electrocardiographic signs of myocardial ischemia; (3) Critically ill patients under close monitoring; (4) Resting SBP > 180 mmHg or DBP > 110 mmHg; (5) Hemodynamic instability or hypotension, including abnormal blood pressure response (significant and symptomatic changes in blood pressure when standing or during exercise); (6) Severe arrhythmias (resting heart rate > 120 beats/minute, uncontrolled atrial fibrillation or ventricular tachycardia > 15/100 beats, high-degree atrioventricular block); (7) Uncontrolled heart failure or cardiogenic shock; (8) Severe complications, including a body temperature exceeding 38°C, acute myocarditis or pericarditis, uncontrolled diabetes or angioedema, thrombosis, or embolism; (9) Aneurysm or severe valve disease or aortic stenosis; (10) Cyanotic congenital heart disease; (11) Obstructive hypertrophic cardiomyopathy; (12) Severe pulmonary hypertension; (13) Acute systemic illness or liver and kidney disease; (14) Digitalis or other drug poisoning; (15) Abnormal surgical incisions; (16) Severe anemia or electrolyte imbalance; (17) Exercise-induced bronchospasm; (18) Marked mental stress or patients who do not understand or cooperate with rehabilitation treatment.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集：本研究采用电子病历系统对患者在院，再入院病历资料进行记录。 数据管理：对于二次入院不在本院的患者，采用电话随访的方式并将数据录入数据库。本研 究通过问卷星平台在线收集相关评估和结局指标数据，再由问卷星导出至 Excel， 结合电子病历系统建立数据库。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data Collection: This study records patient inpatient and readmission data using an electronic medical record system. Data Management: For patients readmitted to a different facility, data is collected through telephone follow-up and entered into a database. This study collects relevant assessment and outcome data online through the Questionnaire Star platform and then exports it to Excel. The database is established by combining this data with the electronic medical record system.
