研究目的:

1. 整理自身免疫性肝炎病例无创临床数据资料,设计合理的生物统计学研究方案并筛选关键因子,建立能有效早期预警自身免疫性肝炎诊断的新数学模型。 2. 验证新数学模型有效性。  

Objectives of Study:

1. Collate the non-invasive clinical data of autoimmune hepatitis cases, design a reasonable biostatistics research scheme and screen the key factors, and establish a new mathematical model which can effectively early warning the diagnosis of autoimmune hepatitis. 2. Verify the validity of the new mathematical model.

药物成份或治疗方案详述:

筛选关键因子:细化风险因子及数据收集,借鉴原有AIH诊断标准及现有研究进展所示的数据类型,据此建立能有效早期预警自身免疫性肝炎诊断的新数学模型 整理数据:收集整理近15年不明原因肝功能异常病例临床数据资料,除了原有AIH诊断标准的数据类型外,加入脾脏厚度、纤维蛋白原水平、Y球蛋白百分百、血三系细胞水平等 建立模型:在建模组通过单因素及多因素分析影响因子,建立拟合度较好的Nomogram预测模型 验证模型:在验证组通过计算ROC、Clibration plots、Decision curve analysis(DCA)间接判断建立的模型的鉴别能力、准确性及临床效能性;同时算出截断值计算灵敏度和特异度,直接验证建立的模型的临床实用性,从而选出最优的模型 

Description for medicine or protocol of treatment in detail:

Screening key factors: refine the risk factors and data collection, draw lessons from the original AIH diagnostic criteria and the data types shown by the existing research progress, and then establish a new mathematical model that can effectively early warning the diagnosis of autoimmune hepatitis. Collate the clinical data of cases with unexplained liver function abnormalities in the past 15 years, in addition to the data types of the original AIH diagnostic criteria, The spleen thickness, fibrinogen level, Y globulin 100% and blood ternary cell level were added to establish the model: in the model group, a Nomogram prediction model with good fit was established by analyzing the influencing factors of single factor and multi-factor. In the verification group, the discriminative ability, accuracy and clinical efficacy of the model were indirectly judged by calculating ROC, Clibrationplots and Decisioncurveanalysis (DCA). At the same time, the sensitivity and specificity of the cutoff value are calculated, the clinical practicability of the model is directly verified, and the optimal model is selected. 

纳入标准:

1. 2005年至2019年上海市第六人民医院感染科就诊的所有不明原因肝功能异常患者。
2. 所有患者均有完善的无创临床检查数据,并均已接受肝穿刺活检。

Inclusion criteria

1. All patients with unexplained abnormal liver function treated in the Department of Infectious Diseases of Shanghai sixth people's Hospital from 2005 to 2019.
2. All patients have perfect non-invasive clinical examination data and have received liver biopsy.

排除标准:

1. 合并有心功能不全、妊娠等影响肝功能的系统性病理、生理状态。
2. 有肝炎病毒标志物阳性符合慢性病毒性肝炎诊断以及临床符合诊断酒精性或药物性肝病诊断。信息不全或缺失的患者。
3. 受试者要求撤回知情同意。

Exclusion criteria:

1. Patients with systemic pathological and physiological conditions affecting liver function such as cardiac insufficiency and pregnancy.
2. Patients with positive hepatitis virus markers, consistent with the diagnosis of chronic viral hepatitis and clinical consistent with the diagnosis of alcoholic or drug-induced liver disease;
3. Patients with incomplete or missing information;
4. Patients requesting withdrawal of informed consent.
