研究目的：

我国的血脂异常患者患病率高，目前血脂管理存在的问题：患者对血脂风险及药物的认知度低，导致治疗依从性低、血脂总体控制不佳，国内药师未参与共享决策相关工作，缺乏共享决策药学服务模式的相关研究。本项目基于共享决策理念探索临床药师工作模式。通过调研现状、构建决策辅助工具，建立由药师主导的共享决策模式，以血脂管理为例，通过科室试点、院内专家评议、院外专家论证、实用推广逐渐建立共享决策模临床药学服务模式，加强慢病管理人才队伍建设，缓解医疗压力，规范药学服务工作，提高我国临床药师慢病管理综合服务能力。  

Objectives of Study：

The incidence rate of dyslipidemia in our country is high, and there are currently problems in blood lipid management: patients have low awareness of blood lipid risks and medicine, leading to low treatment compliance and poor overall blood lipid control. Domestic pharmacists have not participated in shared decision-making related work, and there is a lack of research on shared decision-making pharmaceutical service models. This project explores the working mode of clinical pharmacists based on the concept of shared decision-making. By conducting research on the current situation and constructing decision support tools, a shared decision-making model led by pharmacists is established. Taking lipid management as an example, a shared decision-making model clinical pharmacy service model is gradually established through departmental pilot projects, internal expert evaluations, external expert demonstrations, and practical promotion. The goal is to strengthen the construction of a talent team for chronic disease management, alleviate medical pressure, standardize pharmaceutical service work, and improve the comprehensive service capacity of clinical pharmacists in chronic disease management in China.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）20-85岁住院患者； （2）药师通过查阅电子病历系统，根据《中国成人 ASCVD 总体发病风险评估流程图》进行评估，血脂未达标患者。

Inclusion criteria

(1) Hospitalized patients aged 20-85
(2) The pharmacist evaluated patients with blood lipids that did not meet the standard according to the "Overall Risk Assessment Process for ASCVD in Chinese Adults" by consulting the electronic medical record system.

排除标准：

①经评估无法与病人实现有效沟通和定期随访的；
②恶性肿瘤或血液病放化疗周期未结束的；
③长期卧床病人无亲属长期照护的；
④近三个月内参加其他临床试验的；
⑤不签署知情同意的；
⑥未按时参与随访。

Exclusion criteria：

① Unable to achieve effective communication and regular follow-up with patients after evaluation
② Malignant tumors or hematological diseases whose radiotherapy and chemotherapy cycles have not ended
③ Long term bedridden patients without long-term care from relatives
④ Participated in other clinical trials within the past three months
⑤ Not signing informed consent
⑥ Not participating in follow-up on time.
