研究目的：

本研究的目的是采集不超过80例手术后的废弃包皮组织，在椎元医学技术（上海）有限公司进行工艺开发和质量研究，从而建立完整的工艺规程和质量标准，为后续成纤维细胞注射液项目奠定科学和工艺基础。 同种异体成纤维细胞注射液开发项目，目的是通过成纤维细胞诱导退变椎间盘纤维化维持椎间稳定。主要技术手段为提取成纤维细胞，使用其对治疗不同程度椎间盘退行性病变，以达到恢复椎间盘高度，维持椎间稳定的效果，从而降低腰背痛。成纤维细胞在椎间盘作用，通过成纤维细胞分泌的各类细胞因子诱导退变髓核细胞向纤维化细胞分化，同时通过成纤维细胞自身增殖作用的共同影响，形成纤维化椎间盘。在术后的影像学上，椎间盘可以维持良好的高度，腰椎的整体力线也能获得稳定，降低由于腰椎不稳引起的腰椎疼痛，具有巨大的未被满足的临床需求。  

Objectives of Study：

The purpose of this study is to collect no more than 80 cases of discarded foreskin tissue after surgery, and conduct process development and quality research at Zhuanyuan Medical Technology (Shanghai) Co., Ltd. to establish complete process procedures and quality standards, laying a scientific and technological foundation for subsequent fibroblast injection projects. The development project of allogeneic fibroblast injection aims to maintain intervertebral stability by inducing degenerative disc fibrosis through fibroblasts. The main technical means are to extract fibroblasts and use them to treat different degrees of intervertebral disc degeneration, in order to achieve the effect of restoring intervertebral disc height and maintaining intervertebral stability, thereby reducing back pain. Fibroblasts act on intervertebral discs, inducing the differentiation of degenerative nucleus pulposus cells into fibrotic cells through various cytokines secreted by fibroblasts. At the same time, through the joint influence of fibroblast proliferation, fibrotic intervertebral discs are formed. In postoperative imaging, the intervertebral disc can maintain good height, and the overall force line of the lumbar spine can also be stabilized, reducing lumbar pain caused by lumbar instability, which has a huge unmet clinical demand.

药物成份或治疗方案详述：

一、筛选与入组： 志愿者或其法定监护人于组织采集前14天内签署知情同意书，获得志愿者筛选号，等待医院安排手术。 二、组织采集与清洗 组织采集：志愿者完成包皮切除术后，研究医生或护士立刻收集切除下来的包皮组织，用无菌纱布擦拭包皮表面，用无菌钳持组织，先用 75%酒精充分冲洗，然后用 500 mL 生理盐水冲洗干净；若样本组织污染严重，可在酒精冲洗前先用碘伏消毒处理。将消毒好的包皮组织放于无菌托盘。 组织清洗： 用无菌镊夹取包皮组织，垂直放入生理盐水洗涤液中，尽量减少与瓶口及外部的接触，荡洗 10-20次。观察组织有无可见污物，如有可适当增加荡洗次数。完成洗涤后，夹取组织垂直放入储运瓶中，确保储运液没过组织表面，旋紧储运瓶盖子，并封上封口膜。由带有温度记录的冷链转运至指定的地址。 样本检测：取样本采集清洗后的生理盐水洗涤液，送至医院检验科检验 “沙眼衣原体和淋病奈瑟菌”。若检测结果阳性，由合作方销毁本批次样本。 三、出组： 由于本试验采集的包皮组织属于医疗废物，不影响志愿者的正常诊疗，志愿者完成组织采集后即可出组。 

Description for medicine or protocol of treatment in detail：

1 Filtering and Grouping: The volunteer or their legal guardian shall sign an informed consent form within 14 days before the organization of the collection, obtain a volunteer screening number, and wait for the hospital to arrange the surgery. 2 Organizational collection and cleaning Tissue collection: After the volunteer completes the circumcision surgery, the research doctor or nurse immediately collects the excised foreskin tissue, wipes the surface of the foreskin with sterile gauze, holds the tissue with sterile forceps, rinses thoroughly with 75% alcohol, and then rinses thoroughly with 500 mL of physiological saline; If the sample tissue is severely contaminated, it can be disinfected with iodophor before rinsing with alcohol. Place the sterilized foreskin tissue on a sterile tray. Tissue cleaning: Use sterile forceps to remove the foreskin tissue, place it vertically in a saline washing solution, minimize contact with the bottle mouth and the outside, and rinse 10-20 times. Observe the tissue for visible dirt, and increase the frequency of swabbing appropriately if any. After washing, clamp the tissue vertically into the storage and transportation bottle to ensure that the storage and transportation liquid does not cross the surface of the tissue. Tighten the lid of the storage and transportation bottle and seal it with a sealing film. Transported from a cold chain with temperature records to a designated address. Sample testing: Take a sample and collect the washed physiological saline washing solution, and send it to the hospital's laboratory for testing for "Chlamydia trachomatis and Neisseria gonorrhoeae". If the test result is positive, the partner shall destroy this batch of samples. 3 Outgroup: Due to the fact that the foreskin tissue collected in this experiment belongs to medical waste and does not affect the normal diagnosis and treatment of volunteers, the volunteers can leave the group after completing the tissue collection. 

纳入标准：

1)充分了解试验目的、性质、方法，自愿作为志愿者，并签署知情同意书； 2)年龄在0-18周岁（不含18周岁）之间的男性，采取非激光类的传统包皮环切手术； 3)能够与研究者作良好的沟通，并且理解和遵守本项研究的各项要求。

Inclusion criteria

1) Fully understand the purpose, nature, and method of the experiment, volunteer voluntarily and sign an informed consent form; 2) Men aged between 0 and 18 years (excluding 18 years old) undergo non laser traditional circumcision surgery; 3) Able to communicate well with researchers and understand and comply with the requirements of this study.

排除标准：

1)手术要求的检查项（肝炎三对半【甲、乙、丙肝】、HIV、TPPA【梅毒】）、体格检查结果异常，且研究者判断为有临床意义者； 上述各项检查指标的具体检测项为： ?甲型感染病毒抗体测定（HAV-IgM）； ?乙型肝炎病毒表面抗原测定（HBsAg）； ?乙型肝炎病毒表面抗体测定（HBsAb）； ?乙型肝炎病毒核心抗原测定（HBeAg）； ?乙型肝炎病毒e抗体测定（HBeAb）； ?乙型肝炎病毒核心抗体测定（HBcAb）； ?丙型肝炎病毒抗体测定； ?梅毒螺旋体特异抗体测定； ?人免疫缺陷病毒抗体测定。 2)有临床表现异常需排除的疾病，包括但不限于神经系统、血液、内分泌、免疫系统、泌尿系统、呼吸系统、代谢及骨骼、精神系统、恶性肿瘤等疾病； 3)有传染病、寄生虫、家族遗传病病史； 4)有药物滥用史或使用过毒品者； 5)其它研究者判断不适宜参加的志愿者。

Exclusion criteria：

1) The examination items required for surgery (hepatitis three and a half [A, B, C], HIV, TPPA [syphilis]), abnormal physical examination results, and the researcher's judgment as clinically significant; The specific testing items for the above inspection indicators are: Hepatitis A virus antibody assay (HAV-IgM); Hepatitis B virus surface antigen assay (HBsAg); Hepatitis B virus surface antibody assay (HBsAb); Hepatitis B virus core antigen assay (HBeAg); Hepatitis B virus e antibody assay (HBeAb); Hepatitis B virus core antibody assay (HBcAb); Hepatitis C virus antibody testing; Determination of Treponema pallidum specific antibodies; Human immunodeficiency virus antibody assay. 2) Diseases with abnormal clinical manifestations that need to be excluded, including but not limited to diseases of the nervous system, blood, endocrine, immune system, urinary system, respiratory system, metabolism and bone, mental system, malignant tumors, etc; 3) Have a history of infectious diseases, parasites, or family inherited diseases; 4) Individuals who have a history of drug abuse or have used drugs; 5) Volunteers judged unsuitable by other researchers to participate.
