研究目的：

本研究旨在评估恶性梗阻性黄疸患者术前和术中隐匿性细菌移位（occult-bacterial translocation, OBT）发生率及其对手术后感染性并发症的发生率（postoperative infectious complications, POICS）的影响，以及此类患者接受高侵袭性外科手术对术中发生OBT的影响。  

Objectives of Study：

The purpose of this study was to evaluate the incidence of preoperative and intraoperative OBT and its effect on POICS in patients with malignant obstructive jaundice, and the effect of highly invasive surgery on intraoperative OBT in such patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1:患者无感染相关临床症状和体征：入院至术前体温监测<37.3℃，血常规白细胞计数4~10×10^9/L，中性粒细胞比例正常、精神意识正常、PCT正常； 2:患者有梗阻性黄疸临床表现，如皮肤粘膜黄染、巩膜黄染、皮肤瘙痒、心动过缓、尿色深、粪便颜色变浅或呈白陶土色； 3:血清总胆红素升高，以直接胆红素升高为主，丙氨酸转氨酶、天冬氨酸转氨酶、碱性磷酸酶和γ-谷氨酰转移酶等指标升高或正常，尿胆红素水平增高，粪胆原减少或消失； 4:影像学检查，包括超声、CT或MRCP显示肝胆胰恶性疾病导致的梗阻性胆管扩张； 5:主管外科医师评估需要进行外科手术解除梗阻。

Inclusion criteria

1:No infection-related clinical symptoms and signs were found in the patient: body temperature monitoring was < 37.3C from admission to pre-operation, WBC count was 4~10×10^9/ L, neutrophil ratio was normal, mental consciousness was normal, and PCT was normal. 2:the patients had clinical manifestations of obstructive jaundice, such as yellow staining of skin and mucosa, yellow staining of sclera, skin itching, bradycardia, dark urine, lighter feces or white clay. 3:Serum total bilirubin increased, mainly direct bilirubin, alanine transaminase, aspartate aminotransferase, alkaline phosphatase and γ-glutamyltransferase increased or normal, urinary bilirubin increased, fecal bilirubin decreased or disappeared. 4:Imaging examination, including ultrasound, CT or MRCP, showing obstructive bile duct dilatation caused by malignant hepatobiliary and pancreatic diseases. 5:The competent surgeon assessed the need for surgery to relieve the obstruction.

排除标准：

1:年龄<18岁； 2:无法耐受手术者； 3:术前行化疗和/或放疗、或免疫药物、或靶向药物治疗； 4:半年内有其它腹部手术史。

Exclusion criteria：

1:Age is <18 years old 2:Those who cannot tolerate surgery 3:Preoperative chemotherapy and / or radiotherapy, or immune drugs, or targeted drug therapy 4:There is a history of other abdominal operations within half a year.
