研究目的：

国内外有用右美托咪定治疗老年重症患者术后瞻望的病例报道，但都使用了负荷剂量，本文旨在通过观察老年手术患者应用维持剂量右美托咪定术中镇静，围手术期炎症反应和应激反应与术后认知功能的影响，为降低PND的发生，了解其可能发生的机制提供理论参考。  

Objectives of Study：

Dexmedetomidine has been used in the treatment of postoperative viewing in elderly patients with severe diseases both at home and abroad, but all of them used load dose. This paper aims to observe the effects of maintenance dose dexmedetomidine on intraoperative sedation, perioperative inflammatory response and stress response and postoperative cognitive function in elderly patients undergoing surgery, so as to reduce the occurrence of PND. Understanding its possible mechanism provides theoretical reference.

纳入标准：

纳入标准：实施ERCP手术的老年患者；ASA分级II~Ⅲ级；年龄＞65岁；能够独立完成术前各项认知功能测试。

Inclusion criteria

Inclusion criteria: elderly patients undergoing ERCP surgery; ASA Grade II~ III; Age > 65 years old; Able to complete all preoperative cognitive function tests independently.

排除标准：

排除标准： （1）未获得患者的书面知情同意;（2）在同一住院期间接受第二次择期手术的患者；（3）合并精神系统疾病；（4）严重肝肾功能障碍，严重高血压、糖尿病及冠心病病史；（5）嗜酒，滥用药物，术前简易精神状态检查(Mini Mental Status Examination,MMSE)评估为术前认知障碍者；（6）患者或家属要求退出研究。

Exclusion criteria：

Exclusion criteria: (1) the patient's written informed consent was not obtained; (2) Patients who received a second elective operation during the same hospital stay; (3) combined with mental system diseases; (4) A history of severe liver and kidney dysfunction, severe hypertension, diabetes and coronary heart disease; (5) Alcoholic and drug abusers were assessed as preoperative cognitive disabled by Mini Mental Status Examination (MMSE); (6) Patients or family members request withdrawal from the study.
