研究目的：

1. 比较经肠系膜上静脉肝外门体分流术（TEPS）与内镜治疗对于门静脉血栓/海绵样变患者EGVB的预防效果。 2. 比较TEPS与内镜治疗用于PVT/CTPV患者EGVB预防的安全性，探索两种治疗手段各自的优劣及适应条件。  

Objectives of Study：

1. Compare the preventive effect of transmesenteric extrahepatic portosystemic shunt (TEPS) and endoscopic treatment for esophagogastric varices bleeding (EGVB) in portal vein thrombosis (PVT) or cavernous transformation of portal vein (CTPV) patients. 2. Compare the safety of TEPS and endoscopic treatment for EGVB prevention in PVT/CTPV patients, and explore the advantages, disadvantages, and adaptation conditions of each treatment method.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.18-75岁； 2.诊断PVT/CTPV的患者，内镜下证实EGVB； 3.肝功能Child-Pugh评分＜10分； 4.影像学显示肠系膜上静脉 (superior mesenteric vein, SMV)主干完全或部分通畅； 5.知晓手术及内镜治疗相关风险，签署知情同意。

Inclusion criteria

1. Age 18 to 75 years; 2. Diagnosed PVT/CTPV, confirmed EGVB based on endoscopy examination; 3. Child-Pugh score is less than10; 4. Completely or partially unobstructed SMV according to imaging examination; 5. Understand the risk associated with surgery and endoscopic treatment, and sign informed consent.

排除标准：

1.合并非静脉曲张型消化道出血（急性糜烂性胃炎、消化性溃疡、胃癌等）； 2.近1年内因食管胃底静脉曲张破裂出血行内镜下治疗; 3.既往曾行TIPS术、外科断流手术或其他腹腔手术； 4.合并内科治疗无法控制的自发性腹膜炎； 5.EGVB破裂出血发作未控制或病情稳定时间<1周； 6.肝功能Child-Pugh评分≥10分； 7.术前合并感染、休克等无法耐受侵入性操作及治疗； 8.心脏、肺、肾、脑等重大脏器功能不全及肺动脉高压； 9.米兰标准以外的肝癌及其他恶性肿瘤； 10.孕妇及哺乳期妇女。

Exclusion criteria：

1. Complicated with non-variceal gastrointestinal bleeding (acute erosive gastritis, peptic ulcer, gastric cancer); 2. Previous endoscopic treatment for EGVB within the past year; 3. Previous TIPS, surgical disconnection, or other abdominal surgeries; 4. Complicated with spontaneous peritonitis that cannot be controlled by combined medical treatment; 5. Uncontrolled EGVB or stabilized for less than one week; 6. Child-Pugh score is greater than or equal to 10; 7. Complicated with infection and shock before operation that cannot tolerate invasive procedures and treatment; 8. Dysfunction of organs including heart, lung, kidney and brain, and pulmonary hypertension; 9. Liver cancer and other malignant tumors beyond Milan criteria; 10. Pregnant and lactating women.

The investigators randomly assigned patients to receive either TEPS or endoscopic sequential therapy (1:1 ratio) based on a computer-generated randomization list. Randomization was stratified according to whether the bleeding was first-time or multiple previous bleeding (whether they had been treated with endoscopy or not).
