Objectives of Study：

To explore the application effect of "hospital-community-family" triple linkage cardiac rehabilitation in patients with acute myocardial infarction after PCI

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合美国心脏协会制定的AMI诊断标准; 2.首次行PCI; 3.无精神障碍，具有良好的理解和沟通能力; 4.年龄≥18岁; 5.居住在三个社区卫生服务中心所在社区(与徐州市中心医院合作); 6.患者或其家属签署书面知情同意书

Inclusion criteria

1.met the AMI diagnostic criteria made by the American Heart Association; 2.underwent PCI for the first time; 3.absence of mental disorders and had good understanding and communication skills; 4.aged ≥18 years old; 5. lived in the communities where three community health service centers (in cooperation with Xuzhou Central Hospital); 6. the patients or their family members signed written informed consent.

排除标准：

1.病历不完整； 2.伴有严重的肝、肾、肺功能疾病； 3.有精神病史或者严重并发症的患者； 4.无法参加术后随访或中途退出研究； 5.参与其他心脏康复干预项目。

Exclusion criteria：

1.incomplete medical records were; 2. severe heart, liver, kidney and other important organ dysfunction; 3.patients with a history of mental illness or serious complications; 4. inability to participate in postoperative follow-up visits or withdraw from the study midway; 5.participated in other cardiac rehabilitation intervention projects.
