研究目的：

对心脏手术患者数据进行多因素分析，并借助经脾动脉多普勒血流阻力指数验证心脏手术患者术后感染预测模型。探索经脑-脾轴传导的脾神经交感兴奋情况导致术后感染的可能，识别出更具有特征的术后感染危险因素，有利于围术期制定有效的干预措施来预防此类患者出现术后感染从而使患者受益。  

Objectives of Study：

By conducting a multifactorial analysis of post-operative cardiac patients and utilizing the splenic artery Doppler flow resistance index, we established a predictive model for post-operative infection. We investigated the potential of splenic nerve sympathetic excitation through the brain-spleen axis to cause post-operative infection, identified more characteristic risk factors for post-operative infection, and facilitated the development of effective intervention strategies to prevent such infections and benefit patients during the perioperative period.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.拟行心脏手术患者；2.年龄≥18周岁；3.ASA 分级为I-Ⅳ级；4.签署知情同意书。

Inclusion criteria

1. Patients who intend to undergo cardiac surgery;2. Age≥18 years old;3. ASA is graded I-IV;4. Sign the informed consent form.

排除标准：

1.患者及家属拒绝参与研究；2.房颤；3.重度主动脉反流；4.术前需机械循环支持；5.术前主动脉球囊反博；6.既往移植手术或使用免疫抑制药物；因感染性心内膜炎需手术的患者；孕妇及哺乳期妇女；术前已诊断为感染性疾病的患者；7.存在TEE禁忌症，例如患者拒绝、活动性上消化道出血、食管梗阻或狭窄、食管占位性病变、食管撕裂和穿孔、食管憩室、食管裂孔疝、先天性食管畸形、食管手术后不久、咽部脓肿、咽部占位性病变；8.最近3个月内参与其他临床试验。9.病例脱落（包括退出和中止）;10.因术中死亡或其他原因导致多普勒脾血流测量及TNF-α数据均缺失。

Exclusion criteria：

1. Patients and their families refuse to participate in the research;2. Atrial fibrillation;3. Serious aortic reflux;4. Mechanical circulation support is needed before surgery;5. Preoperative aortic balloon anti-bology;6. Previous transplants or use of immunosuppressants; patients requiring surgery due to infectious endocarditis; pregnant women and lactating women; patients diagnosed with infectious diseases before surgery;7. There are TEE contraindications, such as patient rejection, active upper gastrointestinal bleeding, esophageal obstruction or stenosis, esophageal space-occupying lesions, esophageal tearing and perforation, esophageal diverticulum, esophageal hiatus hernia, congenital esophageal malformation, pharyngeal abscess, pharyngeal space-occupancy lesions;8. Participated in other clinical trials in the last 3 months.9. Case detachment (including withdrawal and suspension);10. Doppler spleen blood flow measurement and TNF-α data are missing due to intraoperative death or other reasons.
