研究目的：

探究通过超声引导下肝门胆管路径经皮经肝穿刺胆道镜治疗胆道狭窄的可行性及安全性，并总结出肝门区路径胆管穿刺的规范流程  

Objectives of Study：

To validate the safety and feasibility of percutaneous transhepatic cholangioscopy (PTCS) in the hepatic hilum by conducting ultrasound-guided PTCS for the treatment of bile duct strictures in the hepatic hilum, and summary a standardized procedure for percutaneous transhepatic cholangioscopy in the hepatic hilum.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①术前经CT、磁共振胰胆管造影、胆道造影等检查确诊为肝门BS或BA；； ②年龄为18-70岁； ③一般情况良好，无重要脏器器质性损害和功能障碍，能耐受全身麻醉和开腹手术者； ④术前肝功能Child-Pugh A级或者Child-Pugh B级； ⑤凝血功能正常； ⑥患者及家属知情同意。

Inclusion criteria

1.Preoperative confirmation of biliary atresia (BA) or biliary stenosis (BS) through CT, magnetic resonance cholangiopancreatography (MRCP), and cholangiography examinations; 2.Age between 18 and 70 years old; 3.General good health, absence of significant organ structural damage or functional impairments, and the ability to tolerate general anesthesia and open surgery; 4.Preoperative liver function categorized as Child-Pugh Class A or Child-Pugh Class B; 5.Normal coagulation function; I6.nformed consent from the patient and their family members.

排除标准：

①患者有重要脏器器质性损害和功能障碍，不能耐受全身麻醉和开腹手术者； ②术前肝功能Child-Pugh C级； ③凝血功能异常； ④急性梗阻性化脓性胆管炎需要急诊手术患者； ⑤伴有肝脏萎缩或者肝内胆管癌变患者。

Exclusion criteria：

1.Patients with significant organ structural damage and functional impairments who cannot tolerate general anesthesia and open surgery; 2.Preoperative liver function categorized as Child-Pugh Class C; 3.Abnormal coagulation function; 4.Patients requiring emergency surgery for acute obstructive suppurative cholangitis; 5.Patients with liver atrophy or intrahepatic bile duct carcinoma.
