研究目的：

探讨缺血性卒中患者外周循环中核苷酸结合寡聚化结构域样受体3（nucleotide-binding leucine-rich repeat proteins 3，NLRP3）炎症小体、下游细胞因子白介素-18 (interleukin-18, IL-18)、白介素-1β（IL-1β）等与胰岛素抵抗指数（Homeostasis model assessment of insulin resistance, HOMA-IR）、血糖及急性期神经功能损伤的临床相关性。  

Objectives of Study：

To investigate the clinical correlation between nucleotide-binding leucine-rich repeat proteins 3 (NLRP3) inflammatory vesicles and their downstream cytokines interleukin-18 (IL-18), interleukin-1 (IL-1β) and insulin resistance index (HOMA-IR), blood glucose and acute-phase neurological impairments in the peripheral circulation of ischemic stroke patients. IL-1β) with clinical correlation with Homeostasis model assessment of insulin resistance (HOMA-IR), blood glucose and neurological impairment in the acute phase.

药物成份或治疗方案详述：

研究分组1：分为轻型组、中型组及重型组。以NIHSS评分患者入院时神经功能缺损程度分为轻型组(NIHSS<5分)、中型组(NIHSS为5-20分)和重型组(NIHSS>20分)。 研究分组2：分为胰岛素抵抗组及非胰岛素抵抗组。以稳态模型胰岛素抵抗指数（HOMA-IR）、胰岛素分泌指数（HOMA-）作为胰岛素抵抗及胰岛素功能的评价指标，使用空腹血糖及空腹胰岛素计算HOMA-IR指数，将患者划分为胰岛素抵抗组和非胰岛素抵抗组。 

Description for medicine or protocol of treatment in detail：

Study group 1: Divided into light group, medium group, and heavy group. According to the NIHSS score, patients were divided into mild group (NIHSS<5 points), moderate group (NIHSS 5-20 points), and severe group (NIHSS>20 points) based on the degree of neurological impairment upon admission. Study group 2: Divided into insulin resistance group and non insulin resistance group. The homeostasis model insulin resistance index (HOMA-IR) and insulin secretion index (HOMA -) were used as evaluation indicators for insulin resistance and insulin function. The HOMA-IR index was calculated using fasting blood glucose and fasting insulin, and patients were divided into insulin resistance group and non insulin resistance group. 

纳入标准：

①符合脑梗死诊断标准； ②经CT或MRI确诊为缺血性脑血管病，排除脑出血； ③患者年龄18≤X≤75岁，男女不限； ④经患者或者其授权委托人同意； ⑤神志清楚，在一定程度上能配合检查。

Inclusion criteria

① Meet the diagnostic criteria of cerebral infarction; ② Diagnosed with ischemic cerebrovascular disease by CT or MRI, excluding cerebral hemorrhage; ③ Patient's age 18 ≤ X ≤ 75 years old, male or female; ④ With the consent of the patient or his/her authorized delegate; ⑤ Clear consciousness and able to cooperate with the examination to a certain extent.

排除标准：

①出血性脑血管病； ②患者或授权委托人不愿配合者； ③严重心肝肾疾病； ④合并自身免疫性疾病； ⑤合并全身或局部感染者； ⑥患者或授权委托人不愿配合者。

Exclusion criteria：

① Hemorrhagic cerebrovascular disease; ② Patients or authorized principals who are unwilling to cooperate; ③ Severe heart, liver and kidney diseases; ④ Combined autoimmune diseases; ⑤ Combined systemic or localized infections; ⑥ Patients or authorized principals who are unwilling to cooperate.
