探讨组蛋白乳酸化修饰与糖尿病认知功能障碍的相关性,及其在预测糖尿病认知功能障碍风险评估中的价值 为糖尿病认知功能障碍“代谢-表观遗传-免疫”级联反应机制的新视角提供一定的佐证  

Objectives of Study:

To investigate the correlation between histone lactate modification and diabetic cognitive impairment and its value in predicting the risk of diabetic cognitive impairment It provides some evidence for the new perspective of the "metabolism-epigenetic-immunity" cascade mechanism of diabetic cognitive dysfunction

药物成份或治疗方案详述:

研究计划总入组患者例数300例,依据2011年WHO的2型糖尿病诊断标准,认知功能评定标准,MoCA 评分<26分者评定为认知功能障碍。根据入选和排除标准进行受试者筛选,将入组患者分为:糖尿病认知障碍组(DCI组)150例、糖尿病组(DM组)150例。收集患者空腹静脉血,用Western blot 检测PBMC中泛组蛋白乳酸化修饰以及H4K12乳酸化修饰水平。从我院病历管理系统收集患者姓名、住院号、年龄、性别、文化水平、糖尿病病程、家族史、用药情况等;实验室检查:乳酸(Lac)、空腹血糖 (FPG)、糖化血红蛋白(HbA1c)、甘油三酯(TG)、总胆固醇(TC)、血谷丙转氨酶(ALT)、谷草转氨酶(AST)、尿酸(UA)、血肌酐(Cr)、尿素氮(BUN)等指标;计算体质数(BMI)。 

Description for medicine or protocol of treatment in detail:

The total number of patients enrolled in the study was 300. According to the diagnostic criteria of type 2 diabetes and cognitive function assessment criteria of WHO in 2011, those with MoCA score < 26 were assessed as cognitive dysfunction. Subjects were screened according to inclusion and exclusion criteria, and the enrolled patients were divided into 150 diabetic cognitive impairment group (DCI group) and 150 diabetic mellitus group (DM group). Fasting venous blood was collected from the patients, and the levels of pan-histone lactation and H4K12 lactation in PBMC were detected by Western blot. The patient's name, hospitalization number, age, gender, educational level, diabetes course, family history and medication status were collected from the medical record management system of our hospital. Laboratory tests: Lactic acid (Lac), fasting blood glucose (FPG), glycated hemoglobin (HbA1c), triglyceride (TG), total cholesterol (TC), serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), uric acid (UA), serum creatinine (Cr), urea nitrogen (BUN) and other indicators; Calculate body mass index (BMI). 

纳入标准:

入选标准 ①≥65岁的成年人 ②符合 2 型糖尿病诊断:依据2011年WHO的2型糖尿病诊断标准 ③认知功能评定标准:MoCA 评分<26 分者评定为认知功能障碍 ④同意参加本研究

Inclusion criteria

Inclusion criteria ① Adults ≥65 years old Meet the diagnosis of type 2 diabetes: According to the WHO diagnostic criteria for type 2 diabetes in 2011 ③ Cognitive function assessment criteria: MoCA score < 26 was assessed as cognitive dysfunction ④ Agree to participate in the study

排除标准:

排除标准 ①近3个月内有严重低血糖、DKA、HHS、乳酸酸中毒等急性并发症; ②伴有其他内分泌疾病,如甲状腺功能亢进症、甲状腺功能减退症; ③明确诊断的神经系统疾病史,如 AD、PD、脑部外伤、中枢神经系统感染、癫痫等; ④近1年内神经影像学证实的脑血管意外; ⑤精神发育迟滞、精神病史,或近3个月有精神药物服用史; ⑥自身免疫病和急慢性炎症性疾病; ⑦休克、心力衰竭、血液病和肺功能不全等导致组织严重缺氧疾病; ⑧合并全身性疾病,如肿瘤、贫血、严重感染等。 ⑨有酗酒、吸毒史,服用导致乳酸堆积的药物或长期应用糖皮质激素等。 ⑩视觉和听觉缺陷难以完成神经病学量表测试。

Exclusion criteria:

Exclusion criteria ① Acute complications such as severe hypoglycemia, DKA, HHS, lactic acidosis in the past 3 months; ② Accompanied by other endocrine diseases, such as hyperthyroidism, hypothyroidism; (3) Clearly diagnosed neurological disease history, such as AD, PD, brain trauma, central nervous system infection, epilepsy, etc. ④ Cerebrovascular accidents confirmed by neuroimaging within the last 1 year; ⑤ Mental retardation, psychiatric history, or a history of taking psychotropic drugs in the past 3 months; ⑥ Autoimmune diseases and acute and chronic inflammatory diseases; ⑦ Shock, heart failure, blood diseases and pulmonary insufficiency lead to severe hypoxia diseases; ⑧ Combined with systemic diseases, such as tumor, anemia, serious infection, etc. ⑨ Have a history of alcoholism, drug use, taking drugs that cause lactic acid buildup or long-term use of glucocorticoids. ⑩ Visual and hearing impairments difficulty completing neurological scale tests.
