研究目的：

通过前瞻性的病例随机对照研究，分析胸腔镜下上肺叶切除术中保留或松解下肺韧带对非小细胞肺癌患者术后康复的临床意义  

Objectives of Study：

To analyze the clinical significance of preserving or releasing lower lung ligaments during thoracoscopic upper lobectomy for postoperative rehabilitation of patients with non-small cell lung cancer in a prospective randomized controlled case study.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）根据肺癌NCCN指南及排除手术禁忌症后符合接受胸腔镜下左/右肺上叶切除术适应症的NSCLC 患者； （2）年龄＜75 岁； （3）术前胸部CT未提示下肺韧带旁淋巴结肿大； （4）患者知情同意并签署知情同意书； （5）术前肺功能检查：FEV1＞1.5L或经过爬楼试验可上5层楼梯； （6）术前 ASA 评分 I-II。

Inclusion criteria

(1) NSCLC patients eligible for thoracoscopic left/right upper lobectomy according to NCCN guidelines for lung cancer and exclusion of contraindications; (2) Age < 75 years old; (3) Preoperative chest CT did not indicate the enlargement of inferior lung paramentamentous lymph nodes; (4) The patient gives informed consent and signs informed consent; (5) Preoperative pulmonary function examination: FEV1 > 1.5L or up to 5 flights of stairs after climbing test; (6) Preoperative ASA score I-II.

排除标准：

（1）术前胸部CT提示第9组淋巴结肿大，不除外转移； （2）同时合并同侧其他肺叶结节，需同时行其他肺叶手术； （3）胸部手术史（仅包括胸腔内手术，不包括乳腺切除术等体表手术）； （4）术前有慢性咳嗽病史； （5）术前合并有肺结核、支气管扩张症、COPD、哮喘等肺相关疾病病史； （6）过去 3 个月内有不稳定性心绞痛或心肌梗死病史； （7）过去 3 个月内有脑梗死或脑出血病史； （8）术中因各种原因中转开胸

Exclusion criteria：

(1) Preoperative chest CT indicated lymph node enlargement in group 9, with no exception of metastasis; (2) Concurrent with other lung lobe nodules on the same side, other lung lobe operations should be performed simultaneously; (3) History of thoracic surgery (including intrathoracic surgery only, excluding body surface surgery such as mastectomy); (4) Preoperative history of chronic cough; (5) Preoperative history of pulmonary tuberculosis, bronchiectasis, COPD, asthma and other lung-related diseases; (6) A history of unstable angina pectoris or myocardial infarction within the past 3 months; (7) Have a history of cerebral infarction or cerebral hemorrhage within the past 3 months; (8) Intraoperative conversion to thoracotomy due to various reasons

After the experiment is completed in September 2026, the data will be made public and can be obtained by contacting the email address of the person in charge.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

①病历报告表(CRF)：每个入选志愿者都必须完成CRF表，完成的原始CRF表为申办单位所拥有，未经研究小组同意，不得以任何形式提供给第三者。 ②数据库的建立：所有数据将采用计算机独立地进行双份录入，由软件自动核对并根据核对结果进行相应的修改。在此期间，将有疑问表通过临床监查员转交研究者进行数据审核，研究者应尽快回答并返回。随机抽取10％的CRF表人工检查一遍，考察是否存在数据出错。确认所建立的数据库正确后，由主要研究者、数据管理人员和统计分析人员对数据进行锁定。锁定后的数据文件不允许再作变动。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Medical Record Report Form (CRF) : Each selected volunteer must complete the CRF form. The completed original CRF form is owned by the sponsor and shall not be provided to a third party in any form without the consent of the study group. 2. Establishment of the database: all data will be independently computer double input, by the software automatically check and according to the check results of the corresponding modification. During this period, the questionnaire will be forwarded to the investigator for data review by the clinical supervisor. The investigator should answer and return the questionnaire as soon as possible. 10% of CRF tables are randomly selected for manual check to see if there are any data errors. After confirming that the established database is correct, the principal investigator, data managers and statistical analysts lock the data. Locked data files cannot be changed.
