研究目的:

糖尿病是重大公共卫生问题,糖尿病前期是糖尿病高危人群进展到2型糖尿病的过渡状态 。糖尿病前期人群数量众多,每年约5%进展为2型糖尿病。临床研究显示,对糖尿病前期人群 进行及时干预,能够避免或延缓其进展为糖尿病,降低并发症风险。糖尿病前期是糖尿病防治 的关键窗口期,然而实践中存在管理与干预均缺乏针对性、持续性等问题,导致效果不佳。综 合分析已有研究与实践,发现急需两个关键技术,一是符合灵敏性、特异性、依从性、经济性 的筛查工具,二是具有成本效果的糖尿病前期社区干预模式。本研究将首先研究两大技术,即 基于机器学习研制糖尿病前期筛查工具,开展社区干预研究并进行成本效果评价;其次,总结 筛查工具与社区干预研究,形成涵盖“筛查+管理+干预”的基于社区的糖尿病前期健康管理模 式,并构建推动模式运行的实现机制;最后,形成可推广的健康管理模式及实施策略。研究成 果将为我国做好糖尿病的一级预防提供技术支撑与决策依据。目前,本研究仅聚焦于符合灵敏性、特异性、依从性、经济性的糖尿病前期筛查工具研制进行临床注册。  

Objectives of Study:

Diabetes is a major public health problem, and prediabetes is a transition from diabetes to type 2 diabetes. The number of prediabetic population is large, with about 5% progressing to type 2 diabetes per year. Clinical studies have shown that timely intervention in prediabetic people can avoid or delay their progression to diabetes and reduce the risk of complications. Prediabetes is the key window period of diabetes prevention and treatment. However, there are problems that management and intervention are lack of targeted and sustainable in practice, resulting in poor results. Through comprehensive analysis of existing research and practice, it is found that two key technologies are urgently needed: one is the screening tool that meets sensitivity, specificity, compliance and economy, and the other is the cost-effective community intervention model of prediabetes. According to the two major technologies, this study will first develop prediabetes screening tool based on machine learning, conduct community intervention research and evaluate the cost-effectiveness of the intervention. Secondly, the study will summarize the screening tools and community intervention model and form community-based prediabetes health management model covering "screening + management + intervention", and the implementation mechanism to promote operation of the model. Finally, the study will form the generalizable health management model and implementation strategy. This study will provide technical support and decision-making basis for the primary prevention of diabetes in China.

Inclusion criteria

Subjects aged between 35 and 74 years who underwent a routine physical examination at the Wusong Hospital Physical Examination Centre of Zhongshan Hospital affiliated to Fudan University

排除标准:

在复旦大学附属中山医院吴淞医院体检中心进行常规体检的35~74岁受检者中: 1、自报糖尿病史的受检者,包括2型糖尿病、1型糖尿病、糖尿病前期 2、自报正在服用降糖药或接受其他降糖治疗的受检者

Exclusion criteria:

Among the subjects aged 35-74 years who underwent routine physical examination at the Wusong Hospital Physical Examination Centre of Zhongshan Hospital affiliated to Fudan University: 1. Subjects with self-reported history of diabetes mellitus, including type 2 diabetes mellitus, type 1 diabetes mellitus, pre-diabetes mellitus 2.Self-reported to be taking glucose-lowering drugs or receiving other glucose-lowering treatments

本研究以糖代谢异常(或2型糖尿病或糖尿病前期)作为阳性诊断标准,分别形成变量组合,针对本研究已研制的筛查工具进行诊断效能评价,主要包括:灵敏度、特异度、阳性预测值、阴性预测值、正确率、约登指数、受试者工作特性曲线下面积。每项指标的含义如下: 1、灵敏度:灵敏度是指在实际患2型糖尿病/糖尿病前期的受检者中经筛查工具筛查标准判断为真阳性所占的比例,反映筛查工具发现病人的能力。 2、特异度:特异度是指在实际未患2型糖尿病/糖尿病前期的受检者中经筛查工具筛查标准判断为真阴性所占的比例,反映筛查工具鉴别排除病人的能力。 3、阳性预测值:阳性预测值是指经筛查工具筛查标准判定为患2型糖尿病/糖尿病前期的受检者中实际患病的受检者所占的比例。 4、阴性预测值:阴性预测值是指经筛查工具筛查标准判定为未患2型糖尿病/糖尿病前期的受检者中实际未患病的受检者所占的比例。 5、正确率:正确率是指筛查工具筛查标准判定的结果与经金标准诊断结果一致所占的比例,反映筛查工具正确区分患者与非患者的能力。 6、约登指数:约登指数表示筛查工具发现真正患者和非患者的总能力,取值范围在0~1之间,约登指数越大,真实性越高。 7、受试者工作特性曲线下面积:曲线往下与坐标轴围成的面积,即为曲线下面积(Area Under Curve,AUC),其取值范围为0-~1,可定量比较模型的预测效果。

Index test:

In this study, abnormal glucose metabolism (or type 2 diabetes mellitus or pre-diabetes mellitus) was used as a positive diagnostic criterion, and combinations of variables were formed separately to evaluate the diagnostic efficacy for the screening tool that has been developed in this study, which mainly includes: sensitivity, specificity, positive predictive value, negative predictive value, correctness, Jordon's index, and the area under the curve of the working characteristics of the subjects. The meaning of each index is as follows: 1. Sensitivity: Sensitivity refers to the proportion of subjects who are actually suffering from type 2 diabetes/pre-diabetes that are judged to be true positives by the screening criteria of the screening tool, reflecting the ability of the screening tool to detect patients. 2. Specificity: Specificity refers to the proportion of subjects who do not actually have type 2 diabetes/pre-diabetes that are true negative by the screening criteria of the screening tool, reflecting the ability of the screening tool to identify and exclude patients. 3. Positive predictive value: Positive predictive value refers to the proportion of subjects with Type 2 diabetes/pre-diabetes who are actually ill as judged by the screening criteria of the screening tool. 4. Negative predictive value: The negative predictive value refers to the proportion of subjects who do not have type 2 diabetes/pre-diabetes, as determined by the screening criteria of the screening tool, who actually do not have the disease. 5. Correct rate: Correct rate refers to the proportion of results determined by the screening criteria of the screening tool that are consistent with the diagnosis made by the gold standard, reflecting the ability of the screening tool to correctly distinguish between patients and non-patients. Yoden index: Yoden index indicates the total ability of the screening tool to find real patients and non-patients, the value range is between 0 and 1, the larger the Yoden index, the higher the authenticity. 7Area Under Curve of Subjects' Work Characteristics: The area surrounded by the curve downward and the coordinate axis is Area Under Curve (AUC), with a value range of 0-~1, which can be used to quantitatively compare the predictive effect of the model.
