研究目的：

开展基于BCW理论联合回授法的社区健康教育方案，评估该方案对社区空巢老年高血压合并糖尿病患者的血压水平、血糖水平及高血压和糖尿病的自我管理能力的影响，从而为改善兰州市社区空巢老年高血压合并糖尿病患者的血压及血糖水平、提高患者与健康相关的生活质量提供参考依据。  

Objectives of Study：

To carry out a community health education program based on BCW theory combined with teach-back method, and to evaluate the effect of the program on the blood pressure level, blood glucose level and self-management ability of hypertension and diabetes in community empty-nest elderly patients with hypertension and diabetes, so as to provide reference for improving the blood pressure and blood glucose level of empty-nest elderly patients with hypertension and diabetes in Lanzhou community and improving their health-related quality of life.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥60 岁的兰州市常住居民。②符合《中国 2 型糖尿病防治指南（2020年版）》的诊断标准，③符合《中国高血压防治指南（2018 年修订版）》的诊断标准。④无子女和保姆照顾，夫妻同住或一人独居。⑤意识清楚，有一定读写能力，无交流障碍。⑥自愿参加本项目。

Inclusion criteria

①The permanent residents aged ≥ 60 years old in Lanzhou. ② in line with the diagnostic criteria of the ' Chinese Guidelines for the Prevention and Treatment of Type 2 Diabetes ( 2020 Edition ) ', ③ in line with the diagnostic criteria of the ' Chinese Guidelines for the Prevention and Treatment of Hypertension ( 2018 Revision ) '. ④No children and nannies to take care of, husband and wife live together or live alone. ⑤clear consciousness, have a certain ability to read and write, no communication barriers. ⑥voluntarily participate in the project.

排除标准：

①正在或近期参加其他相关研究者。②严重并发症，导致沟通障碍、活动障碍、生活不能自理者。③有明确精神病史

Exclusion criteria：

① being or recently participating in other related researchers. ② serious complications, leading to communication disorders, activity disorders, unable to care for oneself. ③have a clear history of mental illness.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

在数据采集过程中，分配未知分组情况的研究人员进行数据的采集，并保证数据的真实有效。用EpiData3.1进行双人录入数据，对有异议的数据核查原始纸质问卷，确定无误后更正。对中途脱落的受试者进行记录，并注明原因，以便后期分析报告。若脱落率＞20%，继续扩大样本量进行数据收集。对录入的数据按照编号的顺序进行归档保存，对原始数据库、分析过程、分析结果，编码记录以及说明文件进行分类保存，并进行多个备份保存于不同的磁盘上。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

During the data collection process, researchers with unknown grouping situations were assigned to collect data and ensure that the data were authentic and valid. Two-person data entry was performed with EpiData 3.1, and the original paper questionnaire was verified for disagreeable data and corrected after determining that there were no errors. Record the subjects who fell off in the middle of the process, and indicate the reason for it, in order to analyze the report at a later stage. If the dropout rate was >20%, continue to expand the sample size for data collection. The entered data were filed and saved in numbered order, and the original database, analytical procedures, analytical results, coding records, and explanatory documents were categorized and saved on different disks with multiple backups.
