研究目的:

骨质疏松症是老龄化社会常见病,肾虚精亏是骨质疏松症的基本病机,肾为先天之本,脾为后天之本,运用补肾健脾的原则组方的中药能够有效防治原发性骨质疏松症及其伴随的脆性骨折,已经获学术界广泛认可,且有多种新药应用于临床。如何科学理解与评价肾、脾所代表的先后天的生物学基础,理解肾、脾之间的关系对骨修复重建的调控环节,具有重要的临床价值和科学意义。肠道微生物是人体中数量最多,种类最丰富和作用机制最复杂的微生物群,在机体内分泌调节中发挥着重要作用,是潜在的健康药物库。本研究推测,肠道菌群紊乱(脾虚)可能加剧绝经后性激素分泌改变(肾虚),促进绝经后骨质疏松症的发生发展。运用肠道基因组学等前沿技术,开展肾虚证、脾虚证代谢特征分析,解析中医“先、后天”理论的生物学基础;并开展“肠道调控”有效中药方剂的生物学基础研究,给出中医证候生物学研究的新证据,将有助于丰富中医药的科学认识。  

Objectives of Study:

Osteoporosis is a common disease in an aging society. Kidney deficiency and deficiency of essence are the basic pathogenesis of osteoporosis. The kidney is the innate root, while the spleen is the acquired root. Traditional Chinese medicine formulated based on the principle of tonifying the kidney and strengthening the spleen can effectively prevent and treat primary osteoporosis and its accompanying brittle fractures. It has been widely recognized by the academic community, and there are multiple new drugs applied in clinical practice. How to scientifically understand and evaluate the biological basis represented by the kidney and spleen, and understand the relationship between the kidney and spleen in regulating bone repair and reconstruction, has important clinical value and scientific significance. Intestinal microbiota is the most abundant, diverse, and complex microbial community in the human body, playing an important role in endocrine regulation and serving as a potential health drug reservoir. This study speculates that intestinal microbiota disorder (spleen deficiency) may exacerbate changes in sexual hormone secretion after menopause (kidney deficiency), promoting the occurrence and development of postmenopausal osteoporosis. Utilizing cutting-edge technologies such as intestinal genomics to analyze the metabolic characteristics of kidney deficiency and spleen deficiency, and to analyze the biological basis of the "first, then" theory in traditional Chinese medicine; And conducting research on the biological basis of effective traditional Chinese medicine prescriptions for "intestinal regulation", providing new evidence for the biological research of traditional Chinese medicine syndromes, will help enrich the scientific understanding of traditional Chinese medicine.

纳入标准:

骨质疏松病例纳入标准 (1)年龄为50-70岁的女性,自然绝经且绝经时间在1年以上者。 (2)骨密度检查提示,腰椎第2-4椎体骨密度均值或髋部骨密度值或股骨颈骨密度中的任何一个部位减少2.5个标准差以上。 (3)符合中医肾虚或脾肾虚诊断。 (4)自愿作为受试对象,并签署知情同意书。 健康受试者入选标准 (1)年龄为18-30岁的健康女性。 (2)骨密度正常。 (3)饮食规律、作息正常。 (4)自愿作为受试对象,并签署知情同意书。

Inclusion criteria

Inclusion criteria for cases of osteoporosis (1) Women aged 50-70 who have experienced natural menopause and have experienced menopause for more than 1 year. (2) The bone density examination indicates a decrease of more than 2.5 standard deviations in the mean bone density of the 2nd to 4th vertebrae of the lumbar spine, hip bone density, or femoral neck bone density at any one location. (3) Conforming to the diagnosis of kidney deficiency or spleen kidney deficiency in traditional Chinese medicine. (4) Volunteer to be the subject and sign an informed consent form. Inclusion criteria for healthy subjects (1) Healthy women aged 18-30. (2) The bone density is normal. (3) Eat regularly and have a normal routine. (4) Volunteer to be the subject and sign an informed consent form.

排除标准:

骨质疏松病例排除标准 (1)有心血管、造血系统和神经精神系统等严重原发性疾病的患者。 (2)在近3个月内接受激素替代(HRT)或降钙素治疗,或近6个月内有连续15天及以上时间接受双膦酸盐治疗的患者。 健康受试者排除标准 (1)近3个月内服用任何药物及保健品。 (2)近期有胃肠道症状。

Exclusion criteria:

Exclusion criteria for cases of osteoporosis (1) Patients with severe primary diseases such as cardiovascular, hematopoietic, and neuropsychiatric systems. (2) Patients who have received hormone replacement (HRT) or calcitonin treatment within the past 3 months, or have received bisphosphonate treatment for 15 consecutive days or more within the past 6 months. Exclusion criteria for healthy subjects (1) Take any medication or health products within the past 3 months. (2) Recently, there have been gastrointestinal symptoms.
