研究目的:

探索一种基于深度学习、能与 EIT 肺灌注评估金标准(高渗盐水注射法)具有强一致性、能最大程度抑制呼吸信号和噪声干扰的 EIT 灌注信号分析方法,以 满足临床对实时、连续、安全、无创、高精度和高稳定性监测肺血流灌注的需求  

Objectives of Study:

The purpose of this study is to explore an EIT perfusion signal analysis method based on deep learning, which has strong consistency with the gold standard of EIT pulmonary perfusion evaluation ( hypertonic saline injection method ) and can suppress respiratory signals and noise interference to the greatest extent, so as to meet the clinical needs of real-time, continuous, safe, non-invasive, high-precision and high-stability monitoring of pulmonary perfusion.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1: 接受机械通气、循环稳定的 ICU 患者或者已完成麻醉诱导插管但尚未 开始手术的患者 2: 年龄大于 18 岁且小于 80 岁; 3: 有中心静脉置管和胸部 CT 扫描

Inclusion criteria

1: ICU patients receiving mechanical ventilation with stable circulation or post-operation patients 2: age∈[18,80] 3: Central Venous Catheterization& chest CT

排除标准:

1: 呼吸系统合并症:气胸、胸腔积液、心包积液、慢性阻塞性肺部疾病、 急性呼吸窘迫综合征、原发性/继发性肺动脉高压和可疑肺栓塞患者等; 2: 近期心脏及胸科术后、胸壁完整性受到破坏的患者; 3: BMI 大于 30kg/m2的肥胖患者; 4: 安置临时或永久心脏起搏器等存在潜在电信号干扰的患者; 5: 电解质明显紊乱:高钠血症,高氯血症,低钾血症等; 6: 凝血功能紊乱有溶血倾向的患者; 7: 存在自主呼吸的患者

Exclusion criteria:

1: Respiratory complications: pneumothorax, pleural effusion, pericardial effusion, chronic obstructive pulmonary disease, acute respiratory distress syndrome, primary/secondary pulmonary hypertension, and suspected pulmonary embolism 2: Patients with recent cardiac and thoracic surgery and impaired chest wall integrity 3: Obese patients with a BMI greater than 30 kg/m2 4: Placement of patients with potential electrical interference such as temporary or permanent cardiac pacemakers 5: Significant electrolyte disturbances: hypernatremia, hyperchloremia, hypokalemia, etc 6: Patients with coagulation disorders and a tendency to hemolysis 7: Patients with spontaneous breathing
