研究目的：

燥湿化痰健脾方是总结全国老中医药专家、广西名中医吕军影教授脑病诊治学术经验而形成治疗脑卒中并发肺部感染的经验方。本研究以脑卒中气管切开肺部感染痰湿蕴肺证患者作为研究对象，采用随机对照试验方法，观察燥湿化痰健脾方治疗前后患者疗效指标（临床肺部感染评分、降钙素原、C-反应蛋白、中性粒细胞与淋巴细胞比值、IL-6、肺泡灌洗液细胞学和病原菌培养、尿代谢组学、拔管成功率、中医证候疗效评分）和安全性指标，客观评价该方对脑卒中气管切开肺部感染痰湿蕴肺证患者的临床疗效，为进一步探讨其疗效机制和推广应用价值提供证据。  

Objectives of Study：

The prescription for removing dampness, resolving phlegm and strengthening spleen is a prescription based on the academic experience of Professor Lv Junying, a veteran Chinese medicine expert and famous traditional Chinese medicine practitioner in Guangxi, in the diagnosis and treatment of cerebral diseases and the treatment of stroke-associated pulmonary infections. This study adopts a randomized controlled trial method to observe the efficacy indicators (clinical pulmonary infection score, procalcitonin, C-reactive protein, neutrophil-to-lymphocyte ratio, IL-6, cytology and pathogenic bacteria culture of alveolar lavage fluid, uric metabonomics, success rate of extubation, and TCM syndrome efficacy score) and safety indicators of the prescription for drying dampness, resolving phlegm, and strengthening spleen before and after treatment, in patients with stroke tracheotomy-induced pulmonary infection with the syndrome of accumulation of phlegm and dampness in the lung. To provide evidence for further exploring its therapeutic mechanism and application value.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)年龄在18-80岁； (2)符合脑卒中的西医诊断标准； (3)符合肺部感染的西医诊断标准； (4) 符合脑卒中中医诊断标准和痰湿蕴肺证的中医证候诊断标准； (5)已行气管切开留置气管套管； (6)脑卒中后出现肺部感染，并符合肺部感染的诊断标准； (7)患者生命体征基本平稳，取得患者或家属同意参加受试。

Inclusion criteria

(1) Aged 18-80 years old; (2)Meet the diagnostic criteria for stroke in Western medicine; (3) Meet the diagnostic criteria for pulmonary infection in Western medicine; (4) Meet the diagnostic criteria for stroke in traditional Chinese medicine and the diagnostic criteria for the syndrome of phlegm-dampness accumulating in the lung; (5) Tracheotomy has been performed and a tracheal tube has been left in place; (6) Pulmonary infection after stroke and meeting the diagnostic criteria for pulmonary infection; (7) The patient's vital signs are basically stable, and the patient or family members agree to participate in the trial.

排除标准：

(1)不符合上述纳入标准； (2)脑卒中前已经存在肺部感染性疾病者； (3)由颅脑损伤、脑肿瘤、脑寄生虫病、脑动静脉畸形、血液系统疾病等引起的脑病； (4)合并全身其他系统感染性疾病者； (5)合并心、肝、肾、内分泌及造血系统等严重原发性疾病者； (6)妊娠或哺乳期妇女，精神疾患者； (7)对本研究已知药物成分过敏或有使用禁忌者。

Exclusion criteria：

(1) Non Conformance the above inclusion criteria; (2) Patients with pulmonary infectious disease before stroke; (3) Brain diseases caused by brain injury, brain tumors, brain parasitic diseases, brain arteriovenous malformations, blood system diseases, etc.; (4) Patients with other systemic infectious diseases; (5) Patients with severe primary diseases such as heart, liver, kidney, endocrine and hematopoietic systems; (6) Pregnant or lactating women, or mental illness; (7) Patients who are allergic or have contraindications for the known drug components in this study.

采用CRF表数据人工录入。根据CRF核查计划的要求内容采用人工检查。核查的内容包括数据的完整性、一致性、逻辑性和方案的违背和偏离等。如纳入排除标准检查、缺失值检查、时间窗检查、逻辑关系检查及实验室检查判断情况检查。如有疑问，可填写数据疑问表，由研究者对疑问表中的问题进行书面解答并签名，交回数据管理员。由数据管理员逐项核对，予以修正确认。上述工作完成后，关闭数据。数据关闭后即可进行数据审核，数据库通过审 核并认为所建立的数据库正确后，将由主要研究者、申办者、统计分析人员和监督管理人员对数据进行锁定。锁定后的数据文件不允许再作变动。数据库将交统计分析人员按统计计划书要求进行统计分析。当数据使用和共享时，需要明确数据使用的目的、方式和范围，并遵守数据共享的相关法律和伦理规定，确保数据的安全和保密。此外，需要明确数据保留期限，并建立相应的数据备份和恢复机制，以保证数据的完整性和可复制性。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Manual entry of CRF table data is used. Manual inspection is conducted according to the requirements of the CRF verification plan. The content of verification includes the completeness, consistency, logicality, and violation and deviation of the data and the plan. Such as inclusion criteria checks, missing values checks, time window checks, logical relationship checks, and laboratory diagnostic checks. If there are any questions, please fill out the data query form, and the researcher will provide a written answer to the questions in the form, sign it and return it to the data administrator. The data administrator will check and correct the data one by one. After the above work is completed, close the data. After the data is closed, data review can be conducted. After the database is reviewed and deemed to be correct, the data will be locked by the main researchers, sponsors, statistical analysts, and supervisory personnel. Locked data files cannot be changed. The database will be handed over to the statistical analysis personnel for statistical analysis according to the requirements of the statistical plan. When using and sharing data, it is necessary to clarify the purpose, method, and scope of data use, and comply with relevant legal and ethical regulations for data sharing to ensure data security and confidentiality.
