研究目的:

探讨不同糖代谢状态脑结构和功能的影像学特征,为T2DM引起的认知功能障碍提供影像学标志。  

Objectives of Study:

To explore the imaging features of brain structure and function in different glucose metabolic states, and to provide imaging markers for cognitive dysfunction caused by T2DM.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1)根据世界卫生组织(1999)2型糖尿病诊断标准,将、实验组分为空腹血糖受损组、糖耐量减低组、糖尿病组(空腹血糖受损组:6.1mmol/L7.0mmol/L,诊断为T2DM,尚未使用药物治疗的); (2)年龄40-60岁; (3)右利手; (4)受教育年限≥6年; (5)汉密尔顿抑郁量表(HAMD)小于8分; (6)无影响书写的眼部疾患,无语言障碍; (7)无脑出血、大面积脑梗塞、肿瘤、颅脑外伤、甲状腺功能减退、维生素缺乏、贫血和肝肾功能障碍等可能影响认知功能病史; (8)具有清晰意识,了解参与研究内容并签署知情同意书; (9)无磁共振检查禁忌症。 8.2 对照组纳入标准: 性别、年龄、教育程度和优势手相匹配的志愿者。 (1)无糖尿病(3.9mmol/L26分; (4)MMSE分数>24分; (5)年龄40-60岁; (6)右利手; (7)受教育年限≥6年; (8)汉密尔顿抑郁量表(HAMD)<8分: (9)无影响书写的眼部疾患,无语言障碍; (10)无脑出血、大面积脑梗塞、肿瘤、颅脑外伤、甲状腺功能减退、维生素缺乏、贫血和肝肾功能障碍等可能影响认知功能病史; (11)具有清晰意识,了解参与研究内容并签署知情同意书; (12)无磁共振检查禁忌症。

Inclusion criteria

Inclusion criteria for the experimental group: (1) According to the World Health Organization (1999) criteria for diagnosing Type 2 diabetes, the experimental group was divided into three groups: impaired fasting glucose (IFG), impaired glucose tolerance (IGT), and diabetes mellitus (T2DM). IFG was defined as a fasting plasma glucose level of 6.1 mmol/L or higher but less than 7.0 mmol/L, and an OGTT 2-hour value of 7.8 mmol/L or lower; IGT was defined as a fasting plasma glucose level of 6.1 mmol/L or higher but less than 7.0 mmol/L, and an OGTT 2-hour value of 7.8 mmol/L to 11.1 mmol/L; T2DM was diagnosed when FPG > 7.0 mmol/L. The participants in the experimental group were not receiving medication for T2DM at the time of study. (2) Participants were between the ages of 40 and 60. (3) They were right-handed. (4) Their educational years had been ≥ 6 years. (5) Their Hamilton Depression Scale (HAMD) scores were less than 8. (6) They did not have any eye diseases that affected their writing, nor did they have language barriers. (7) They did not have any history of cognitive impairment due to conditions such as cerebral hemorrhage, large-area stroke, tumor, brain injury, hypothyroidism, vitamin deficiency, anemia, or liver and kidney dysfunction. (8) They had a clear understanding of the study content and signed a informed consent form. (9) They did not have any contraindications for magnetic resonance imaging (MRI). The inclusion criteria for the control group were as follows: Gender, age, educational level, and dominant hand matched with volunteers who met these criteria. (1) They did not have diabetes (FPG < 3.9 mmol/L or 7.0 mmol/L, and OGTT 2h < 7.8 mmol/L). (2) They did not have subjective memory loss. (3) Their MoCA score was greater than or equal to 26. (4) Their MMSE score was greater than or equal to 24. (5) They were between the ages of 40 and 60. (6) They were right-handed. (7) They had an educational year count of at least six years. (8) Their HAMD scores were less than or equal to 8. (9) They did not have any eye diseases that affected their writing, nor did they have language barriers. (10) They did not have any history of cognitive impairment due to conditions such as cerebral hemorrhage, large-area stroke, tumor, brain injury, hypothyroidism, vitamin deficiency, anemia, or liver and kidney dysfunction. (11) They had a clear understanding of the study content and signed an informed consent form. (12) They did not have any contraindications for MRI.

排除标准:

(1)排除有明确原因如(脑外伤、脑肿瘤、脑出血、大面积脑梗塞、抑郁/焦虑症、阿尔兹海默氏病、帕金森病痴呆、额颞叶痴呆、克-雅病、服用影响意识药物等)引起的认知功能减退。 (2)排除酒精中毒。 (3)排除肺心病、糖尿病急性并发症、急性感染性疾病、原发性肾病以及甲状腺功能亢进症、心力衰竭、肿瘤等其他重大疾病。 (4)排除脑白质病变:年龄相关白质改变(ARWMC)评分量表评分>1分(ARWMC标准:0分:没有病变,1分:局部病变,2分:病变融合成片,3分:弥漫性累及整个区域,有或者没有累及皮层下U状纤维〉 (5)排除磁共振检查禁忌症。 (6)HAMD量表评分大于8分。

Exclusion criteria:

(1) Exclude cognitive impairment caused by clear causes such as brain injury, brain tumor, cerebral hemorrhage, large-area cerebral infarction, depression/anxiety disorder, Alzheimer's disease, Parkinson's dementia, front temporal lobe dementia, Creutzfeldt-Jakob disease, taking consciousness-altering drugs, etc. (2) Exclude alcohol poisoning. (3) Exclude pulmonary heart disease, acute complications of diabetes mellitus, acute infectious diseases, primary nephropathy, and other major diseases such as hyperthyroidism, heart failure, tumors, etc. (4) Exclude white matter lesions: age-related white matter changes (ARWMC) scoring > 1 point (ARWMC standard: 0 points: no lesion, 1 point: local lesions, 2 points: lesions integrated into a patchwork pattern, 3 points: diffuse involvement throughout the region with or without subcortical U-shaped fibers involved〉 (5) Exclude the contraindications for magnetic resonance imaging. (6) HAMD score > 8 points.
