研究目的：

轻度认知功能障碍（Mild Cognitive Impairment, MCI) 是界于正常衰老认知和痴呆的中间阶段。与正常衰老相比，MCI有较高的风险发展为痴呆。近年来以太极拳（Tai Chi Chuan, TCC) 改善脑功能的研究日益趋多，经颅磁刺激（Transcranial Magnetic Stimulation, TMS）因其无创简便等特点在脑功能研究中具备一定优势，其疗效均有一定的临床证据。但将TMS联合TCC改善MCI患者执行功能的研究目前尚缺乏进一步证据。因此本研究拟将TMS 联合TCC对老年MCI患者进行为期12周的干预。利用fNIRS技术检测干预前后患者大脑前额叶皮层血氧激活情况，以认知功能改善为主要结局指标分析TMS联合TCC对患者执行功能的影响。探究：①TMS联合TCC治疗老年MCI的疗效。②分析患者干预前后执行功能的变化与脑区激活水平的关联性。  

Objectives of Study：

Mild Cognitive Impairment (MCI) is the intermediate stage between normal aging cognition and dementia. Compared to normal aging, MCI has a higher risk of developing dementia. In recent years, Tai Chi Chuan (TCC) has been increasingly used to improve brain function, and Transcranial Magnetic Stimulation (TMS) has certain advantages in brain function research because of its non-invasive and simple features, and its efficacy has some clinical evidence. However, there is a lack of further evidence for combining TMS with TCC to improve executive function in MCI patients. Therefore, in this study, we propose to combine TMS with TCC in elderly patients with MCI for a 12-week intervention. The effect of TMS combined with TCC on executive function was analyzed using the fNIRS technique to detect prefrontal cortical blood oxygen activation before and after the intervention, with improvement in cognitive function as the main outcome indicator. To investigate: (i) the efficacy of TMS combined with TCC in the treatment of MCI in the elderly. (ii) To analyze the correlation between the changes in executive function and the level of activation of brain areas in patients before and after the intervention.

纳入标准：

1)年龄60岁-90岁; 2)平衡功能BBS评分>40分 3)MoCA评分18-26分 4)MMSE评分<24分 5)基本的沟通能力和正常的视听力; 6)身体能力足以进行耐力运动训练; 7)存在主观记忆问题 8)知情同意。

Inclusion criteria

1) Age 60-90 years old. 2)Balance function BBS score >40 3)MoCA score 18-26 4)MMSE score <24 5) Basic communication skills and normal visual and hearing. 6) physical ability sufficient for endurance sports training. 7) Subjective memory problems 8)Informed consent.

排除标准：

1)确诊AD或其他类型的痴呆; 2)家族性早发性痴呆病史; 3)参加任何研究性药物研究; 4)过去2年的癫痫发作史； 5)任何重大精神障碍，包括临床确诊的重度抑郁症、双相情感障碍或精神分裂症; 6)脑损伤的既往史或MRI证据； 7)不稳定性慢性病，包括高血压、低血压、糖尿病、心脏病等； 8)过去10年内有明显的酗酒或药物滥用史; 9)严重或不愈合的伤口、溃疡或骨折； 10)近30年内每周3次，进行中等强度有氧运动训练20分钟以上； 11)颅内置有铁磁物质，心脏起搏器、支架； 12)耳内助听器；

Exclusion criteria：

1) Diagnosis of AD or other types of dementia. 2) Family history of early onset dementia. 3)participation in any investigational drug study. 4)history of seizures in the past 2 years 5)Any major psychiatric disorder, including clinically confirmed major depression, bipolar disorder, or schizophrenia. 6)previous history of brain injury or MRI evidence; 7)unstable chronic medical conditions, including hypertension, hypotension, diabetes mellitus, heart disease, etc; 8)Significant history of alcohol or drug abuse within the past 10 years. 9)severe or non-healing wounds, ulcers or fractures 10)moderate intensity aerobic training for 20 minutes or more, 3 times per week for the last 30 years 11)intracranial placement of ferromagnetic substances, pacemakers, stents 12) hearing aids in the ear;

The assignment group was selected from a sealed opaque envelope provided by the rehab therapist. The envelope contained only two sheets of paper. The letter "E" on one sheet indicated the transcranial magnetic stimulation combined with taijiquan group, and the letter "C" on the other sheet indicated the sham stimulation transcranial magnetic stimulation combined with taijiquan group.
