研究目的：

本项目在高龄社区中进行前瞻性、对照性研究，对老年人进行内在能力评估并分级进行多维度针对性干预，延缓老年人内在能力下降，维护老年人内在能力，探索本土化符合价值医疗的整合照护模式，促进中国的健康老龄化。  

Objectives of Study：

This project proposed a prospective, controlled study in senior communities to assess the intrinsic capacity of older adults and implement multidimensional targeted interventions in a graded manner, delay the decline of the intrinsic capacity of older adults, maintain the intrinsic capacity of older adults, explore localized integrated care models that are consistent with value-based care, and to promote healthy aging in China.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）年龄≥75岁的社区居民；（2）可交流、配合；（3）慢病稳定期，近1月内无慢性疾病急性加重或急症。

Inclusion criteria

(1) community residents aged 75 years or older; (2) Be able to communicate and cooperate with researchers; (3) stable chronic diseases without any "acute episode" or emergencies.

排除标准：

（1）疾病终末期，疾病已经影响患者正常的功能发挥，且不可逆。如静息状态下具有心衰或呼吸困难的症状，腹水或浮肿影响正常活动，器官功能衰竭（充血性心力衰竭[NYHA心功能分级III和IV级]，肝硬化失代偿期或Child-Pugh分级C级，尚需规律放疗或化疗的肿瘤患者）； （2）医方评估其预期生存期有限（如肿瘤晚期）； （3）近期不能参与活动（如：近6个月内曾发生心肌梗塞，不稳定心绞痛，心律失常控制不佳，近6个月内曾发生脑梗并遗留肢体活动障碍，高血压病控制不佳，近期新发的肺栓塞，近3个月内曾发生肢体或脊柱骨折，控制不佳、影响体育锻炼或可因活动加重的骨关节炎、类风湿关节炎等）； （4）进食受限，需要肠内或肠外营养支持的，无法在社区环境下改善营养的老人； （5）失能（卧床）、进展期痴呆。

Exclusion criteria：

(1) Unreversible end-stage diseases status with impaired functional abilities. Including but not limited to dyspnea at rest, ascites or edema which affect daily activities, disease with organ function failure with in the last month, including but not limited to: congestive heart failure with NYHA level III and IV heart function, decompensated cirrhosis or patient with Child-Pugh level C, patients who still need regular chemotherapy or radiotherapy; (2) Expected survival less then 1 year estimated by doctors; (3) Patients not suitable for exercise, including but not limited to myocardial infarction within 6 months, unstable angina, uncontrolled arrhythmia, stroke within the last 6 months with residual obstructed movement, uncontrolled hypertension, newly onset pulmonary embolism, fracture in the past 3 months, uncontrolled arthritis; (4) Those who have difficulties in feeding and require enteral or parenteral nutritional support and are unable to improve nutrition in a community setting;(5)Incapacitated or bed-ridden elderly and elderly with progressive dementia.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

CRF信息采集由患者自评、工作人员评估共同完成。能够熟练使用电子设备与应用的受试者，在工作人员的帮助下自行填写ICOPE筛查问卷与卫生经济学随访问卷，由工作人员对受试者进行内在能力详细评估并记录。对于无法熟练应用电子设备的受试者，由工作人员询问受试者问卷内容并辅助填写。以CRF作为原始记录，填写时间需记录在案，更正记录时不应改变原始记录。由研究团队人员定期对数据进行核查与抽查。 电子采集和管理系统基于泰康公司“养老云平台”，在平台中为每位受试者建立数据记载的表单，由移动端数据采集应用导入或人工手动输入。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

CRF information will be collected by a combination of patient self-assessment and staff assessment. Participants who were proficient in the use of electronic devices and applications will complete the ICOPE screening questionnaire and the health economics follow-up questionnaire on their own with the help of staff, and the detailed subjects' intrinsic capacity in five dimension will be assessed and recorded by staff. For participants who were not proficient in the use of electronic devices, the staff will ask the participants about the content of the questionnaire and assisted them in completing it. The CRF will be used as the original record, and the time of completion will be recorded; the original record should not be changed when correcting the record. The data will be verified and spot-checked by the research team staff regularly. The electronic collection and management system is based on Taikang's "Senior Care Cloud Platform", in which a form is created for each subject to record data, which is imported by the mobile data collection application or manually entered.
