研究目的：

1. 探索VR 技术在ICU患者舒适度的应用效果。 2. 比较VR技术与平板电脑在播放视频对ICU患者舒适度的差异性。 3. 为VR 技术和内容进一步改进提供参考依据。  

Objectives of Study：

Explore the application effect of VR technology on the comfort of ICU patients. Compare the differences in comfort of ICU patients between VR technology and tablet computers when playing videos. Provide reference basis for further improvement of VR technology and content.

药物成份或治疗方案详述：

（1） 成立VR干预管理小组。人数在5~10人之间，参与人员需要有一定的沟通能力和技巧，具备解决VR关键问题的能力，研究方向主要须涉及重症护理或具备使用VR的相关经验。 （2）本研究硬件是低成本、可便携的Google Cardboard VR设备，配合安卓手机来运行视频播放软件，通过头戴式耳机播放声音。本研究基于压力恢复理论，该理论认为自然环境可以通过放松副交感神经系统来促进情绪状态的积极变化和从心理生理压力中恢复。注意恢复理论则表明，自然环境（如风景、植被和水）的视觉暴露具有保护效应，可以恢复生理、情感和注意力功能。选择视频主题内容是自然风光，为3D沉浸式视频，播放时长为6分14秒，缓慢移动镜头下漫游山涧瀑布、大海沙滩、平原溪畔等景色，背景为舒缓的音乐配合白噪声。VR设备与耳机有一次性眼垫、耳机罩，机身用标准医用消毒纸巾进行擦拭。 

Description for medicine or protocol of treatment in detail：

Establish a VR intervention management team with 5-10 members. The participants should have good communication skills and problem-solving abilities for VR-related issues, and their research direction should mainly focus on critical care nursing or have relevant experience in using VR. The hardware used in this study is low-cost and portable Google Cardboard VR devices, which are operated with Android smartphones to play video playback software and use a headset for sound playback. This study is based on the stress recovery theory, which suggests that natural environments can promote positive changes in emotional states and recovery from psychological and physiological stress by relaxing the parasympathetic nervous system. The attention restoration theory also suggests that visual exposure to natural environments such as scenery, vegetation, and water has a protective effect, restoring physiological, emotional, and attentional functions. The selected video content theme is natural landscapes, presented in 3D immersive videos with a playback time of 6 minutes and 14 seconds. The video slowly pans through scenic views such as mountain streams, waterfalls, beaches, and riverbanks, accompanied by soothing music and white noise. The VR devices and headsets are equipped with disposable eye cushions and earphone covers, and the device's body is wiped with standard medical disinfectant wipes. 

纳入标准：

1. 纳入标准 （1）首次入住ICU，年龄≥18周岁的患者； （2）初中以上学历，能积极配合； （3）患者各生命体征稳定、均处于清醒状态，ICU镇静镇痛评估量表（RASS）评分＜±2； （4）预计在ICU中的住院时间≥12小时

Inclusion criteria

Inclusion criteria: (1) Patients aged 18 or above who are admitted to the ICU for the first time; (2) Patients with a junior high school education or above who can actively cooperate; (3) Patients with stable vital signs and are conscious, with a Richmond Agitation-Sedation Scale (RASS) score of less than +/-2; (4) Patients expected to stay in the ICU for at least 12 hours.

排除标准：

（1）耳聋、痴呆、癫痫、昏迷、视力异常； （2）精神病史麻醉及镇静药物影响交流和反应者； （3）不理解普通话者； （5）格拉斯哥昏迷指数（GCS）＜13分； （6）使用谵妄评估量表（CAM-ICU）有谵妄发生。

Exclusion criteria：

(1) Patients with hearing impairment, dementia, epilepsy, coma, or visual impairment; (2) Patients with a history of mental illness or those affected by anesthesia or sedative drugs that affect communication and response; (3) Patients who do not understand Mandarin Chinese; (4) Patients with a Glasgow Coma Scale (GCS) score of less than 13; (5) Patients with delirium confirmed by the Confusion Assessment Method for the ICU (CAM-ICU).

The video content chosen is natural scenery, in the form of a 3D immersive video. The video lasts 6 minutes and 14 seconds, with slow-moving camera shots of landscapes such as mountain streams, waterfalls, beaches, and plains, accompanied by soothing music and white noise. The VR equipment and headphones come with disposable eye pads and earphone covers, and the equipment is wiped down with standard medical disinfectant wipes.

After successful enrollment, the responsible nurse provides relaxation videos based on tablet computers to the control group, while explaining the specific content, duration, and usage of the video. The video content is consistent with that of the VR group, which is about natural scenery with slow moving shots of mountain streams, waterfalls, beaches, plains, and other scenery, accompanied by soothing music and white noise. Routine nursing care measures are also consistent for both groups.
