研究目的：

吞咽障碍是卒中患者最常见的功能障碍之一，缺乏有效的药物治疗，诸多患者接受常规治疗后仍存在咽缩肌无力、咽部残留、渗漏、误吸等问题。目前仍缺乏直接改善咽缩肌肌力的治疗方法，本项目拟通过分析球囊咽腔肌肉抗阻训练对卒中后吞咽障碍是否有效，以及球囊咽腔肌肉抗阻训练与经颅磁刺激联合治疗效果是否优于单一的外周或中枢干预，以期为吞咽障碍患者开展康复训练提供新的治疗方法。  

Objectives of Study：

Swallowing disorder is one of the most common dysfunction in stroke patients, the lack of effective drug treatment, many patients after conventional treatment still have pharyngeal muscle weakness, pharyngeal residue, leakage, aspiration and other problems. Currently, there is still a lack of treatment methods to directly improve the muscle strength of pharyngeal shrinkage muscle. This project intends to analyze whether the resistance training of balloon pharyngeal muscle is effective for swallowing disorders after stroke, and whether the combined treatment effect of balloon pharyngeal muscle resistance training and transcranial magnetic stimulation is superior to the single peripheral or central intervention, in order to provide a new treatment method for the rehabilitation training of patients with swallowing disorders.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①首次发病卒中；②年龄18～80岁之间；③有典型吞咽障碍临床表现，如咽反射减弱或消失、咽喉部力量控制缺失、语调或声音改变、噎呛或咳嗽等；④患者生命体征稳定，不伴严重并发症；⑤无明显认知障碍，可以配合康复训练。

Inclusion criteria

① First stroke; ② Between 18 and 80 years old; ③ There are typical clinical manifestations of dysphagia, such as reduced or disappeared pharyngeal reflex, loss of laryngeal power control, intonation or voice change, choking or coughing, etc. (4) The patient's vital signs were stable without serious complications; ⑤ no obvious cognitive impairment, can cooperate with rehabilitation training.

排除标准：

①体内有植入式电子装置(如心脏起搏器)或颅内有金属植入物； ②存在严重内科疾病、生命体征不稳定；③癫痫；④近期使用过镇静剂；⑤局部皮肤病变。

Exclusion criteria：

①Implantable electronic devices (such as cardiac pacemakers) in the body or metal implants in the brain; ② Serious medical disease and unstable vital signs; ③ epilepsy; ④ recently used sedatives; ⑤ Local skin lesions.
