研究目的：

构建口底及舌癌重建术后吞咽障碍患者的直接摄食训练方案，探讨直接摄食训练方案对口底及舌癌重建术后患者吞咽功能的应用效果。  

Objectives of Study：

Construct a direct feeding training program for patients with swallowing disorders after reconstruction surgery for oral floor and tongue cancer, and explore the application effect of the direct feeding training program on swallowing function of patients after reconstruction surgery for oral floor and tongue cancer.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥18周岁；②手术方式为口底或舌（舌切除>50%或舌根）扩大切除术+游离组织瓣转移修复术+气管切开术；③患者拔除气管套管后，洼田饮水试验Ⅲ～Ⅴ级；④伤口愈合良好；⑤意识清楚，能够自主交流并配合进食增稠食物；⑥患者或家属日常使用微信；⑦知情同意，自愿参加本研究。

Inclusion criteria

① Age ≥ 18 years old; ② The surgical method is expanded resection of the floor of the mouth or tongue (tongue resection>50% or tongue base), free tissue flap transfer repair, and tracheotomy; ③ After the patient removed the tracheal cannula, the Wada drinking water test was conducted at levels III-V; ④ The wound healed well; ⑤ Clear awareness, able to communicate independently and cooperate with eating thickened food; ⑥ Daily use of WeChat by patients or family members; ⑦ Informed consent and voluntary participation in this study.

排除标准：

①既往或同时合并其他影响吞咽功能的疾病，如脑卒中、颅脑损伤、重症肌无力等；②肿瘤有远处转移；③术前已接受放射治疗；④因病情仍需禁食禁水。

Exclusion criteria：

① Previous or concurrent diseases that affect swallowing function, such as stroke, head injury, myasthenia gravis, etc.; ② The tumor has distant metastasis; ③ Received preoperative radiation therapy; ④ Due to illness, it is still necessary to fast and drink.

在对照组的基础上，于基线时，使用食物增稠剂调制低、中、高3种稠度的液体，对吞咽障碍患者进行V-VST测试。若患者能够通过至少1种稠度的食物，则完善吞咽造影检查，选择适宜患者经口进食的食物黏稠度，并由吞咽专科护士调配后指导患者经口进食。

Intervention：

On the basis of the control group, V-VST tests were conducted on patients with swallowing disorders at baseline using food thickeners to prepare liquids of low, medium, and high consistency. If the patient is able to pass at least one consistency of food, complete swallowing imaging examination, select the appropriate food viscosity for the patient to eat orally, and guide the patient to eat orally after being prepared by a swallowing specialist nurse.
