评估基层卫生保健机构实施糖尿病患者诊疗服务与健康管理最佳循证实践指南的障碍和促进因素。  

Objectives of Study：

To evaluate barriers and facilitators to the implementation of best evidence-based practice guidelines for diagnosis, treatment and health management of patients with diabetes in primary health care Settings.

纳入标准：

a.机构： 纳入标准：①机构位于贵州省行政范围内；②开展针对糖尿病患者的慢性病诊疗及健康管理服务；③可以开展健康体检服务。 b.机构负责人： 纳入标准：①属于机构内的职工，合同制或编制内；②对于机构开展糖尿病诊疗流程熟悉；③需入职一年以上。 c.医生： 纳入标准：①具备医师资格证；②参与糖尿病患者的诊疗及健康管理。 e.公卫人员： 纳入标准：①参与糖尿病患者的诊疗及健康管理。 f.患者 纳入标准：①符合国家II型糖尿病诊断标准;②居住在当地基层医疗机构所在社区;③年龄≥35岁。

Inclusion criteria

a. Organization: Inclusion criteria: ① The institution is located within the administrative scope of Guizhou Province; (2) To carry out chronic disease diagnosis and treatment and health management services for diabetic patients; ③ Health examination services can be carried out. b. Head of the organization: Inclusion criteria: ① belong to the organization's staff, contract system or establishment; (2) Familiar with the procedures of diabetes diagnosis and treatment in institutions; ③ Must have been employed for more than one year. c. Doctor: Inclusion criteria: ① Have a doctor's certificate; (2) Participate in the diagnosis, treatment and health management of diabetic patients. e. Public health personnel: Inclusion criteria: ① Participate in the diagnosis, treatment and health management of diabetic patients. f. Patient Inclusion criteria: ① Meeting the national diagnostic criteria for type II diabetes; ② Live in the community where the local primary medical institutions are located; ③ Age ≥35 years old.
