探究母体产前焦虑与产后抑郁、新生儿皮质醇反应是否存在相关性及其剂量依赖关系。  

Objectives of Study：

To explore the correlation between maternal prenatal anxiety and postpartum depression, neonatal cortisol responses and its dose-dependent relationship.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

产妇：计划行择期剖宫产；ASA分级Ⅱ级； 单胎妊娠；37≤孕周<42；18≤年龄≤35周岁；无腰硬联合麻醉禁忌症；自愿参加本次临床试验并签署知情同意书 新生儿：母亲参与研究;1、５minApgar评分均＞７分；生命体征稳定（体温36-37℃、呼吸 40-60次/min、心率110-150次／min）

Inclusion criteria

Maternal: plan to carry out elective cesarean section;ASA grade Ⅱ;Singleton pregnancy;37≤ gestational age <42;18 years old≤ age ≤35 years old;No contraindications to combined spinal-epidural anesthesia;The puerpera volunteered and signed the informed consent form Neonates: Mother participate in the study;1min and 5min Apgar scores > 7 points;Stable vital signs (body temperature 36-37℃, breathing 40-60 times /min, heart rate 110-150bpm)

排除标准：

产妇：既往精神疾病、认知障碍史（焦虑、抑郁除外）；被检测出胎儿先天异常或生长迟缓；孕期有吸烟饮酒史、服用类固醇类药物史；无法正确理解问卷内容，无法与研究人员合作。新生儿：先天性器质性疾病；畸形新生儿；高胆红素症、新生儿溶血症；因病理情况转新生儿科者

Exclusion criteria：

Maternal: previous history of mental illness and cognitive impairment (except anxiety and depression);Congenital abnormalities or growth retardation of the fetus are detected;History of smoking, drinking and steroid use during pregnancy;Inability to properly understand questionnaire content and cooperate with researchers .Neonates: congenital organic disease;Malformed newborn;Hyperbilirubinosis, Hemolysis of newborn;Patients transferred to the neonatology department due to pathological conditions

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

①患者入室后，常规监测，记录基线数据，用预冷的 EDTA塑料管采集2ml静脉血。 ②进行腰硬联合麻醉，在向蛛网膜下腔注入局麻药之前采集脑脊液0.2ml。 ③恢复体位后手术，并采集术中数据；胎儿娩出后，立即从脐带的双钳段中取出 2ml脐动脉血和脐静脉血。 ④产后采集新生儿进行免疫接种前后的唾液及母亲爱丁堡抑郁量表评分等。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

① After the patient entered the room, routine monitoring was performed, baseline data was recorded, and 2ml venous blood was collected with pre-cooled EDTA plastic tube.② After combined spinal-epidural anesthesia,0.2ml of cerebrospinal fluid was collected before local anesthetic was injected into subarachnoid space.(3) After restoring the posture, the operation was performed, the intraoperative data were collected;Immediately after delivery of the fetus, 2ml of umbilical artery blood and umbilical venous blood were removed from the double-forceps segment of the umbilical cord.(4) Postpartum saliva before and after immunization and maternal Edinburgh Depression Scale score were collected.
