研究目的：

心血管病（CVD）既是全球也是我国疾病死亡的首位原因，如何防治CVD，已上升成为重大的公共卫生问题，也是落实健康中国战略的重要任务之一。实践证明，以生活方式干预和危险因素防控为核心的健康管理可有效延缓或避免心血管事件的发生。本研究通过对健康体检中心的体检人群进行CVD高危人群早期筛查，分析该人群CVD高危率以及CVD高危对象的主要危险因素、10年CVD患病风险和心理状态及其变化，同时采用“互联网＋”新型健康管理针对CVD高危对象进行干预及效果评价并提出科学、合理且有效的健康管理策略，以早期识别健康体检CVD高危人群，进行针对性防控，降低CVD发病率、复发率、致残率和死亡率，为CVD早期防治提供理论基础和科学依据。  

Objectives of Study：

Cardiovascular disease (CVD) is not only the leading cause of death in the world but also in China. How to prevent and treat CVD has become a major public health problem, and is also one of the important tasks in implementing the Healthy China strategy. Practice has proved that health management centering on lifestyle intervention and prevention and control of risk factors can effectively delay or avoid the occurrence of cardiovascular events. In this study, the early screening of CVD high-risk population will be conducted on the physical examination population in the health examination center, and the high risk rate of CVD in this population, the main risk factors of CVD high-risk population, the risk of CVD in 10 years, the psychological state and their changes will be analyzed. At the same time, "Internet +" health management will be used to conduct intervention and effect evaluation on high-risk population of CVD, and scientific, reasonable and effective health management strategies were proposed to identify high-risk groups of CVD in early physical examination, carry out targeted prevention and control, and reduce the incidence, recurrence, disability and mortality of CVD, providing theoretical basis and scientific basis for early prevention and treatment of CVD.

纳入标准：

初筛对象入选：本研究初筛对象入选标准包括以下 3 条，须同时满足才能参加筛查。 如有其中 1 项或 1 项以上不符合，则不能参与筛查。
①年龄在 35～75 岁之间的居民; ②四川省常住居民; ③自愿参加且签署《知情同意书》。
CVD 高危对象判定标准: 符合以下两条中的任意一条：
①根据国家 CVD 中心发布的《心血管病高危人群早期筛查与综合干预项目技术方 案》，符合以下两类标准之一:
A.疾病史(符合以下4条中的1条即可)：
a.心肌梗死病史; b.接受经皮冠状动脉介入治疗; c.接受冠状动脉搭桥手术; d.脑卒中病史。
B.血压、血脂(符合以下3 条中的1 条即可):
a.收缩压(SBP)≥160mmHg 或舒张压(DBP)≥100mmHg;b.低密度脂蛋白胆固醇(LDL-C)≥160mg/dL(4.14mmol/L);c.高密度脂白胆固醇(HDL-C)30mg/dl
(0.78mmol /L)。
②根据《2019年中国心血管病风险评估和管理指南》推荐的“China-PAR 模型”对全部筛查对象进行CVD风险评估。如果筛查对象10年CVD风险≥10%，则判定为高危对象。

Inclusion criteria

Selection of primary screening objects: The selection criteria of primary screening objects in this study include the following three items, which must be met simultaneously before screening. If there is one or more non-conformities, they cannot participate in the screening.
(1) Residents aged between 35 and 75;
(2) Permanent residents of Sichuan Province;
(3) Voluntarily participate and sign the Informed Consent Form.
Criteria for high-risk Population of CVD: meet any of the following two criteria:
(1) According to the Technical Plan of Early Screening and Comprehensive Intervention Project for High-risk Population of Cardiovascular Disease issued by the National CVD Center, it meets one of the following two standards:
A. Disease history (one of the following four items can be met):
a. History of myocardial infarction; b. Receiving percutaneous coronary intervention; c. Receiving coronary artery bypass surgery; d. History of stroke.
B. Blood pressure and blood lipids (only one of the following three items can be met):
a. Systolic blood pressure (SBP) >= 160mmHg or diastolic blood pressure (DBP) >= 100mmHg; b. Low density lipoprotein cholesterol (LDL-C) >= 160mg/dL (4.14 mmol/L); c. High density lipoprotein cholesterol (HDL-C) 30mg/dl(0.78mmol /L)
2. According to the "China-PAR Model" recommended by the "Guidelines for Cardiovascular Disease Risk Assessment and Management in China in 2019", CVD risk assessment was conducted for all screening subjects. If the 10-year CVD risk of the screening object is >= 10%, it will be determined as a high-risk Populationt.1)

排除标准：

（1）年龄小于35岁或大于75岁; （2）非四川省常住居民；（3）无法填写问卷或不愿参加本研究者。

Exclusion criteria：

1. The age is less than 35 years old or more than 75 years old;
2. Non-resident residents of Sichuan Province;
3. Those who are unable to fill in the questionnaire or unwilling to participate in this study.
