研究目的：

探究与传统被动物理治疗技术（PP）相比，主动物理治疗技术（AP）是否能更大程度改善慢性肩痛患者的疼痛和功能。  

Objectives of Study：

To explore whether active physiotherapy (AP) can improve pain and function in patients with chronic shoulder pain to a greater extent than traditional passive physiotherapy (PP).

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、年龄＞18岁，不考虑诊断标签，有肩痛（>3个月）的患者。 2、参与者的肩部视觉模拟疼痛（VAS）评分在≥3分以上。 3、能正确理解和使用中文并清楚地表达自己的意愿。

Inclusion criteria

1. Age > 18 years old, regardless of the diagnostic label, with shoulder pain (> 3 months). 2. The visual analogue pain (VAS) scores of the participants were ≥3. 3. Be able to understand and use Chinese correctly and express yourself clearly.

排除标准：

1、物理治疗红旗征：恶性肿瘤、全身性感染、脱位未复位、肩关节不稳、急性肩袖撕裂、神经系统损伤、严重骨质疏松症。 2、其他部位疾病引起肩痛：牵涉痛（心肌缺血，膈肌疼痛）、恶性肿瘤（根尖性肺癌，转移性肺癌）、风湿性多肌痛。 3、其他不适合或无法完成治疗计划的情况，如认知功能的变化、老年性肌萎缩、身体活动、低强度激光治疗、电疗或短波透热等治疗禁忌症。 4、有肩关节骨折或手术史。 5、过去六个月内，接受过肩部物理治疗的患者。

Exclusion criteria：

1. Red flag signs of physical therapy: malignant tumor, systemic infection, dislocation not reduced, shoulder instability, acute rotator cuff tear, nervous system injury, severe osteoporosis. 2. Shoulder pain caused by other diseases: referred pain (myocardial ischemia, diaphragmatic pain), malignancy (apical lung cancer, metastatic lung cancer), rheumatic polymyalgia. 3. Other conditions that are not suitable or unable to complete the treatment plan, such as changes in cognitive function, senile muscular atrophy, physical activity, low-intensity laser therapy, electrotherapy, or short-wave diathermy are contraindications for treatment. 4. A history of shoulder fracture or surgery. 5. Patients who have had shoulder physical therapy in the last six months.

盲法：

所有患者对自己的分组情况不知情，所有研究人员被告知不要在干预或随访时告知患者分组情况。因为手法干预的特殊性，治疗人员不可能施盲。基线和结果评估人员由不参与实验的其他治疗师进行。

Blinding：

All patients were unaware of their grouping status, and all researchers were told not to inform patients of their grouping status during the intervention or follow-up. Because of the particularity of manipulative intervention, it is impossible for the treatment staff to blind. Baseline and outcome evaluators were conducted by other therapists who were not involved in the experiment.
